Can blurry vision be caused by a tear during cataract surgery?

After cataract surgery, I have very blurry vision and painful sensitivity to light and glare.  Can this be caused by a tear during surgery using Iris hooks, and if so, can it be repaired? This problem is for my right eye only.  My left eye was also done and it turned out perfect.  I am healthy and have no other health issues.  The sensitivity to light and glare is causing headaches and pain going down my face to my neck and shoulder.  I also see streaks of light only in my right eye. I can't spend the rest of my life like this.

Get an independent second or third opinion from a different surgery.  Iris hooks don't usually tear the iris and small iris tears are usually neither painful nor blurry. What does the surgeon say is the problem?
