It's like Jodie has written. Can you see the Big E on the lens chart, with correction from lenses, glasses or contacts?
The yardstick for legally blind goes by how well you can see with correction. The information you have given is for making your glasses.
Ask your doctor what your vision is in terms of how you see with glasses/contacts. He can also tell you if you are legally blind.
That is only your glasses RX. You can not tell anything about your vision.
It's like looking at your shoes and trying to tell how fast your run.

What is your vision like when you're wearing your glasses/contacts? Can you read the big "E" on the eye chart? If you can, you're not legally blind.
