Have you been checked out for sleep apnea. Your nighttime struggling for breath sounds a great deal like that.
Myasthenia gravis was and is my concern because when I lay down I feel like something blocks my nasal passage and throat which makes it difficult to breath. I wake up out of a sound sleep struggling to get air.
Anxiety is often caused by high cortisol.
Far less than the number of people that are seeing physicians for "medical" problems and they should be seeing a psychiatrist for anxiety or depression or other psychological problem

There is an unfortunate tendency to ascribe an emotional problem to ailments they cannot diagnose. I wonder how many people are seeing psychiatrists when they should be treated medically.
Find a neuro ophthalmologist near you at www.aao.org and see for an evaluation. Myasthenia gravis needs to be excluded.
