Mandates HMOs (but not other types of insurers) to cover infertility treatment as a preventative benefit. The law does not define "infertility treatment" and HMOs have interpreted the term as excluding IVF.
This is what I found. I would hope not all HMO's exlcude IVF.

I am in IL-BCBS pays for IVF-you only pay a 250 deductible. I am sure your RE office can answer further.

It would be good if u call UR RE office or infertlity office ,they can give little info ,regarding with which insurance RE works,
i did it many times, they were very kind to tell me .
