Nissen fundoplication - extreme pain then no pain and acid reflux again

I had my surgery about 5 weeks ago.  Everything was going great, no acid reflux, no pills, except for the pain at the top right incision site (I had the laproscopic) that got steadily more intense.  Then, around 4 weeks post-op, the pain stopped entirely (i.e., went from horrible to gone in one day).  Then about two days after that, I started to get heartburn and acid-reflux again (although not nearly as bad as before surgery).  

I am worried that the wrap slipped.  Is this possible?  Can they determine if it did slip?  Should I be contacting the doctor or am I over-reacting and should just wait and see?

i had a lap nissen 1 year ago but wonder if some of the symptoms i am having now could be because something is wrong.  i am extremely tired, dont feel like eating, hurts right below xyphoid and very tender to touch, and get nausea after every meal (only 3,4 bites). Could mine have slipped.  He used staples around the esophagus, could one have eroded.
