Chronic constipation in 4 year old?

My granddaughter has been treated for over a year by her pediatrician. Lay off dairy, up the veggies and fruit and Miralax. She is still sobbing in extreme pain (today being day 11 since she's gone poop). My question is this: shouldn't she be referred to a Gastroenterologist by now? My son keeps pushing her pediatrician and they just say no, let's up the Miralax, let's do this or that...well nothing works! Unfortunately he is on CHC which is the insurance for lower income families. I'm about ready to self pay and get this little girl some help. Any advice is greatly appreciated!

Have you tried a suppository my son was a miralax for a long time after a while as well. But for immediate relief

Today was the first time anyone has suggested a glycerine suppository. Her mommy did it tonight and I hope to hear good news in the morning. The best thing is...we went back to the doctor today and saw an on call doc who gave us a referral ASAP. He could not believe we haven't been referred before this! So happy this little one will finally get some relief and we will get some answers!
