Could this be JUST IBS?

Note: This is not me, this is from a female aged 31.

Since 1,5 year ago I have irregular bowel movements, usually 4-5 incomplete bowel movements per day. Always after defecation, I feel like something is stuck in my rectum. Five months ago I went to the ER with acute right lower quadrant pain near the appendix with the pain reflecting right in my back. Normal bloodwork, plain abdominal x-ray showed excessive intestinal gas. Pain continued for several days and then nausea came also.
I couldn't eat anything because everything made me have symptoms. I started therapy with pinaverium bromide (I'm from Europe, it's a medicine for IBS, a spasmolytic) but symptoms got only mildly better. Had an upper & lower abdominal ultrasound which was clear except a hepatic hemangioma of 29mm. Had also therapy with librax but the result was the same. Then came a colonoscopy with a diagnosis of IBS (Spastic sigmoid colon was the major problem).
Started therapy with triavil (Amitryptiline, a TCA antidepressant and Perphenazine, a conventional antipsychotic) in low dose and the only improvement so far is in my stomach, and also the right lower quadrant pain is not continuous. But often still some discomfort. Any ideas??

MEDICAL PROFESSIONAL
Hi there!

Well, without a detailed clinical evaluation it would be difficult to determine the cause of your symptoms. With growths/ masses ruled out, possibilities that may need to be considered include dietary causes, stress/ anxiety, hormonal/ endocrine causes, IBS, primary GI motility issues, infection/ inflammation, malsorption syndromes, metabolic issues, medication side effect etc. I would suggest considering a detailed evaluation by gastroenterologist. After a specific cause is identified, it can be managed accordingly.
Hope this is helpful.

Take care!
