Could i have HIV???

Hi

I had sex with a sex worker in Munich Germany 3 days ago and i was wearing a condom.  in the middle the condom broke and i felt it and i immediately exited (about 3 or 4 second later).  we did exchange a lot of kisses.  Of course i asked the worker several questions.  She is Czech, has only been doing this job for 3 months and was working as a sister nurse previously for 4 years.  she claims that she gets tested but i really cannot depend on that.  She is 22 years old, she had no signs of any rash or anything of that kind.  i checked myself after the incident and i was not hurt or bleeding and my penis did not have any kind of blood on it.

i am now feeling a bit ill and i am getting some weird muscle pains.  I feel weak and i know that i am being extremely paranoid about this and some of these pains im getting could be purely mental.  i was tested about 6 months ago and i was negative and i have had numerous encounters since then but they were all 100% protected.  im really worried that i could have caught something, although i know the probability is very low.

Welcome to our Forum. this was a low risk exposure.  If your partner was contacted through a licensed brothel in Germany, it is likely that she was checked recently  as this is routine for commercial sex workers in Germany.  Even if this was not your situation, your risk is low.  It would still be unlikely that your partner had HIV and even if she did, your risk for infection would be less than 1 infection for every 1000 exposures.

As for your symptoms, they have occurred to soon following your exposure to be due to early HIV infection.  When people become symptomatic from early HIV infection, the symptoms typically begin two week following exposure, not a 3 days.  I would not worry about your symptoms.

You may wish to get tested for HIV and other STDs (which are more common than HIV) following this exposure but there is little to worry about.  You can test for common STDs like gonorrhea and chlamydia at this time and expect reliable results.  For accurate HIV test results, you need to wait until 4 weeks following exposure, at which time you should be tested with a combined HIV p24 antigen/HIV antibody test (these are sometimes referred to as DUO tests).  When you do get tested, if you decide to, I am confident your results will be negative.  

I hope my comments are helpful. Please do not worry and do not give up your commitment to consistent condom use. Despite the occasional mishap, they remain the best means of preventing HIV and STD acquisition.  EWH

You have been mis-informed.  HIV is not spread by kissing, French or otherwise.  Further, even if genital secretions had gotten in your mouth, this does not put you at risk for HIV.  

Your exposure at 70 days did not put you at risk for HIV either.  If you are worrried about the exposure 70 dyas ago, a standard HIV antibody test at this time would give you a definitive andser about that exposure and, I am sure, it would should that you did not get HIV. EWH

i know that i sound very paranoid Dr. Edward but i need to tell u some additional info,

I was in the Ukraine about 70 days ago and i had PROTECTED sex with numerous sex workers, but in one occasion i rubbed my penis on her vagina for a minute or so and i received unprotected oral sex from her, and since then i had a fever from jan 1 to jan 7 and today my sore throat is getting worse and my temperature went up to 38C (another fever).  I am really scared

sorry to bother you again dr but i cant sleep or concentrate at my job or do anything else for that matter.

Also Dr. Edward,

She gave me oral sex with the condom on after i took it out of her and then we kissed, hence she might have had some of her own vaginal fluids on the condom and i kissed her after wards.

Thank you so much for your prompt reply Dr. Edward.  it has put my mind at ease because i was about to loose it!!

I do have another inquiry about the situation.  we exchanged a lot of french kisses and i heard recently that it is very common that HIV is transmitted through the mouth gums and i do have gum problems and they frequently bleed when i brush my teeth...

Also yes it was through a licensed brothel in germany but through my research i found out that testing is not mandatory there.
