You are asking almost every question about STD/HIV risk there is, it seems! You're going to need to do some serious reading; it would take me an hour to answer all your questions in detail. I'll hit some highlights briefly.
Condoms are close to 100% effective prevention against HIV. The 85% figure comes from pregnancy, and does not reflect the protection of a properly used condom anyway. 85% is the "use effectiveness", i.e. the percent protection against pregnancy after you take in to account forgetting the condom entirely, mis-timing sex without a condom, etc. The reason for occasional failures in HIV prevention have more to do with condom breakage; failure to use a condom with a presumed uninfected partner who actually has HIV; and other misinformation by people making the claims, whether intentionally lying or through forgetfulness or misunderstanding.
If a condom doesn't obviously break, a person is protected. Oil based lubricants can lead to rupture, but if that doesn't happen, there is no problem. In other words, oil doesn't cause microscopic leaks.
Yes, of course it is possible for someone to catch HIV the first time exposed, or even during one's first episode of sex. But statistically those outcome are very rare.
HIV testing almost never is recommended after the sorts of exposures you describe, unless a person's partner is known for sure to have HIV or to be at particularly high risk, like a gay/bi man or an injection drug user. Since you are sexually active outside a permanent, committed relationship, just have an HIV test every 1-2 years and don't worry about particular exposures.
HIV is not "everywhere". The large majority of heterosexuals with even hundreds of sex partners, let alone 6-20 partners, never get HIV, at least not in the US and other industrialized countries.
STDs indeed can cause increased mucus, but not the clear "bubble" you describe, usually cloudy or overt pus, dripping from the penis. Your gonorrhea and chlamydia tests show you weren't infected.
Just be sensible in partner selection and use condoms for vaginal or anal sex, outside permanent committed relationships. That's all there is to safe sex. Do those things and you'll go a lifetime without catching HIV.
