DR. HHH M.D

I've read many stories and onlines and scary stuff, but I know the media blows stuff out of proportion. I've read that condoms are only 85% efficient. Is that correct? what's the real risk for heterosexuals male when it comes to HIV? I heard that HIV is hard to get. I read about exposures for HIV being 1/2000 for one unprotected sexual encounter. Is it possible for someone to get HIV in the first encounter. Many people say that if a condom does rip sometimes you won't feel it. Thiers this religiouse pastor who use to be a male escort and he claims he always wore condoms, but came up HIV positive so he is telling people in my neighborhood that condoms are not life savers. I think condoms are 99.9 percent accurate if used properly so how often does the 0.01 happend? I had an hiv test 2 years ago and it was negative. Since then I had two exposures with CSW. After the 1st exposure I got a clear bubble like discharge. It was really small and it didnt come out on its own I had to squeeze it. I got a Chlamydia and Gonnorrhea test and it was negative. That's the only time it came out. I never saw the discharge again after that day. This was 34 days after exposure. Then 4 weeks ago I went to another CSW. I got caught in the moment and didnt notice she put baby oil on her vagina(not the condom). Well sex lasted 5 minutes. After that I squeezed the bottom part of the condom where the sperm is stored and their wasnt any leakage. Does that mean the condom was intatct? I notice white dry strans on the bottom of my penis after I went home but nothing on the head. These 2 sexual exposures are the only ones I've had in 2 years since my last hiv test. Do you reccomend testing from these two exposures? I never got teached anything about HIV. I had a friend that slept with over 6 woman and probably only used pertection with like 20 and he is clean.  I thought the virus was everywhere... Does an std cause more urethral mucus production?
I  know you have 30 years experiance.

Yes, of course.  How could it not be noticed??  Can a balloon pop without being obvious?  Can you drop a glass and not notice whether or not it shatters?

One last thing and I will have no more questions. So if a condom rips the male will notice right? considering I am not drunk or anything.

Thankyou for clearing that up. So many claims on the internet of condoms failing and I presume its Religiouse groups promoting abstinance. Were human and were sexual by nature. You rock doctor !!!  Have a nice New Year. Out of curiosity do you still have a Clinic?

You are asking almost every question about STD/HIV risk there is, it seems!  You're going to need to do some serious reading; it would take me an hour to answer all your questions in detail.  I'll hit some highlights briefly.

Condoms are close to 100% effective prevention against HIV.  The 85% figure comes from pregnancy, and does not reflect the protection of a properly used condom anyway.  85% is the "use effectiveness", i.e. the percent protection against pregnancy after you take in to account forgetting the condom entirely, mis-timing sex without a condom, etc.  The reason for occasional failures in HIV prevention have more to do with condom breakage; failure to use a condom with a presumed uninfected partner who actually has HIV; and other misinformation by people making the claims, whether intentionally lying or through forgetfulness or misunderstanding.

If a condom doesn't obviously break, a person is protected.  Oil based lubricants can lead to rupture, but if that doesn't happen, there is no problem. In other words, oil doesn't cause microscopic leaks.

Yes, of course it is possible for someone to catch HIV the first time exposed, or even during one's first episode of sex.  But statistically those outcome are very rare.

HIV testing almost never is recommended after the sorts of exposures you describe, unless a person's partner is known for sure to have HIV or to be at particularly high risk, like a gay/bi man or an injection drug user.  Since you are sexually active outside a permanent, committed relationship, just have an HIV test every 1-2 years and don't worry about particular exposures.

HIV is not "everywhere". The large majority of heterosexuals with even hundreds of sex partners, let alone 6-20 partners, never get HIV, at least not in the US and other industrialized countries.

STDs indeed can cause increased mucus, but not the clear "bubble" you describe, usually cloudy or overt pus, dripping from the penis.  Your gonorrhea and chlamydia tests show you weren't infected.

Just be sensible in partner selection and use condoms for vaginal or anal sex, outside permanent committed relationships.  That's all there is to safe sex.  Do those things and you'll go a lifetime without catching HIV.

I hope this helps.  Best wishes--  HHH, MD
