Risk Assessment for Gay Male

Hello, doctor. I'll be brief.

I'm a 25 year old gay male. Last HIV test was six months ago. Result: negative.

In the intervening time, I've had encounters with six different men. I performed fellatio on all of these men, swallowing in at least one case. In a single encounter, I was the receptive partner in anal sex. We used a condom and water-based lubricant. I always ask about HIV status; all men said that they were negative.

I'll be going in for an HIV test pretty soon. I always get butterflies in my stomach for these things. So, yeah, I know that doctors don't exactly have crystal balls. But, given what you know, how much would you bet that my result will be negative?

Congratulations on a responsible approach to your sexuality and your commitment to sexual safety.  I'm especially please to hear you routinely have "the talk" about HIV status before sexual activity.

If there are no lapses in these practices, you can expect to go a lifetime without catching HIV.  The only slight uncertainty concenrns unprotected oral sex.  While few if any HIV infections have been attributed to oral-to-penile transmission, there is a slight risk from oral exposure to semen.  But because you discuss HIV status, the odds are good that you will have few exposure to infected men.

If you want a wild stab at numerical odds, I would say the chance of a positive result is around one in a million, based on maybe a 1% chance an unprotected oral partner had HIV and 1 chance in 10,000 you were infected if he did: 0.01 x 0.0001 = 0.000001.  Actually, this may be too high; the chance one of your partners had HIV probably is under 1%.

So regardless of the composition of my gonads, crystal or otherwise, you can definitely expect a negative result on your upcoming HIV test.
