Very worried in europe - pls help

Dear sir, 4.5 wks ago after drunken night out I had unprotected vaginal sex (twice) and reciprocal oral sex with a Romanian callgirl in Amstdam, using viagra. Was first time wt a prostitute. Got full test at 1 wk post exposure whose result after another week was chlamydia. Nurse gave two azitromicines and told me not to have sex for 7 days and to come back after 3 months for another test. After urging, told can come for combo test at 7 wks. At 3 wks went to GP who took a swab. 2 days later he called saying that they have again found chlamydia and gave doxycycline. At 4 wks went back to GP as I still felt discomfort - should have been given 2 tabs per day for 7 days. Now taking these. Called the call girl last week who made no comment on chlamydia but said that she's tested every two mnths and I shouldn't worry about HIV. Next wk will be 5 wks post exposure. My gf is worried why I have not had sex with her (already before the incident). Want to but cant use condoms. Although she is still on pill, we were thinking about trying to start trying for a baby soon. Am really very stressed out. Cant properly take care of my ill parents because of this and my job is being affected. Also I cry whenever I think about my sweet gf and what I did.
1) What are HIV risks?
2) I would like to do Combo test now and not wait for 7 wks. Is there a rapid Combo test? I believe this is available in Rotterdam at a private place. However, they use a finger prick with immediate result and I know from English websites that Duo tests offered in London take a tube of blood and results take 20 mins-4 hrs. Are we talking about the same test? What is accuracy?
3) Will all the chlamyd medicine affect accuracy of HIV test or hide HIV symptoms?
4) When can I start having sex again with my gf? Is chlamydia out of my system/will be by the time this set of tabs finishes? Is nurse's relaxed attitude regarding HIV justified?
5) Which HIV tests should I do? Prefer sthg here with results on the same day.

Your research sounds valid, so it would appear such a test is available.  I'm sure it is highly reliable.  But that's all I can say.  I don't understand why this makes you more worried.  It means your negative test is even more reliable.

That will end this thread.  I won't have any more comments.

Dear Dr,
I read your comment, but I learned from the website that there is a rapid blood test for HIV1/2Ag/Ab Combo test strip, it is from the invernessmedical, the determine HIV1/2 Ag/Ab Combo. it is more known as abbott test strip, we are selling this in China already by some private companies, but the 3rd generation has been sold here for a long long time, could you please check and see if it is true that there is no such combo test strip yet? because I just bought one and did a test myself 26 days after exposure, the result is neg, I am so worried now.
Thank you very much
Jerry

Glad to have helped.  Take care.  And for goodness' sake, start using condoms for casual sex!

Dear Doctor Hunter Handsfield,

Many thanks for your answer, just read it. Thanks for taking the time over the weekend to get back to me so comprehensively. Really appreciate it as I am foreigner (from India) in Holland and I needed to talk to an expert like you in English. Your response provides me with reasurance and I will continue to read it when I feel worried or scared. Btw, the Romanian call girl was 21 years old and said that she doesn't have unprotected sex with everyone. Even if she is lying or doesn't know she has HIV, the HIV chances/risks you describe are so low even though we had sex twice. So the best case scenario is to  learn from this experience and cherish what I have - it's not worth to get drunk, take viagra and do these things! Just need that negative result. So, I will try to be mellow! I will talk to my doctors about chlamydia and get the normal combo test (non-rapid) done at a hospital. I will let you know.

Thank you very much again.

Best wishes to you too.

Welcome to the forum.  You really should have no serious worries about HIV in this situation.  Here is why.

Heterosexually transmitted HIV remains far, far less common than chlamydia and other common STDs; that you apparently caught chlamydia does not imply especially high risk for HIV.  Second, the large majority of sexually active women -- including from eastern Europe and including commercial sex workers -- do not have HIV, and people rarely lie about HIV when asked directly.  Third, even if a partner had HIV, the chance of transmission during any single episode of vaginal sex averages less than 1 in a thousand.

1) Those comments answer question 1; your risk of HIV is less than one chance in many thousand, perhaps around 1 in a million.

2,5) To my knowledge, there is no rapid 4th generation (combo) test.  All the rapid tests are only for antibody.  I see no reason why you are so intent on the rapid result, but if that continues, have any of the available rapid tests about 6 weeks after the last vaginal sex exposure.  However, since the combo test would be 100% reliable at this time (4 weeks or more after the last exposure), even with a few days wait for the result, this would give you a sooner answer than waiting until 6+ weeks for a rapid test.

3) There are no medicines or illnesses that interfere with HIV testing or that delay positive results.  It's an urban myth.  The only possible exceptions are certain life-threatening terminal illnesses and powerful chemotherapy -- and even these are theoretical only, with virtually no reported cases that they actually had any effect on HIV testing.  Certainly chlamydia or its treatment have no effect.

4) From an HIV standpoint, you can safely have unprotected sex with your partner right now.  But if you want that extra measure of reassurance, have a combo test and resume sex with her as soon as you get the negative result.

Whatever testing you choose, stay mellow until then.  You can definitely expect a negative result.

On a slight change of subject, the chlamydia test results you report are very peculiar.  It is extremely unlikely you still were infected after the antibiotics.  You might clarify this with your docto(s).

I hope this helps.  Best wishes--  HHH, MD
