finger cut risk

Dear Dr,
I have read many many previous posts, but most deal with paper cuts.
To the point...I cut my thumb, a  scoop of skin cut off about 5mm long and 3mm wide. It bled and formed a scab.
24 hours later i fingered a CSW & received unprotected handjob, but am worried because i noticed that the scab had a crack in it and i could see into the cut. It was not bleeding at all.
to be honest im not even sure it definatley came into contact with  vaginal secretions.


3 weeks later i have a painful right ear and mild pain on the right of my throat, and one episode of diarrhoea and post nasal drip for 2 days (which has now cleared up)

Is there a risk? i cant bear the anxiety (am testing at 28 days with duo soon)

ps, on a personal note i admire your work (as a fellow, but inexperienced, junior medic)

Welcome to the Forum. A s a reader of our Forum, you what I am about to tell you- there is no risk to this exposure.  No risk at all.  To begin with, you do not know that this person had HIV.  Most commercial sex workers do not have HIV. Secondly, HIV is not spread through superficial contamination of cuts, scrapes and wounds, even recent ones and the wound you describe was 24 hours later.  The healing process which closes of wounds and cuts begins immediately as the bleeding stops, preventing risk for infection.  that your healing wound had a crack in it does not change this.

there is no reason to worry.  The earache, diarrhea and other problems you describe are unrelated coincidence, not a manifestation of HIV.

I realize that you will get tested for your own peace of mind but, form a medical perspective, testing is not even needed in relationship to this exposure.

by the way, i am very anxious as i have been involved in care of HIV+ve patients in my home country and West Africa, I know my worrying must seem stupid but i cant help it
