oral sex please help

Hello Doctor,

here is my situation, 6 weeks ago i went on a trip and I had received unprotected oral sex from a man who im assuming is HIV+... What happened what he rimmed me for maybe 15-30 seconds while was standing up and gave me a blow- job for maybe a minute.

When i got home from my trip the next day i had a very bad burning in the tip of my penis.  so i went to the doctor approx. 10 days later.  he gave me a gonahhra and clymindya test and they both turned out negative.

He prescribed me a 10 day prescription of doxycycline and gave me a shot of antibiotics in the butt.

Its been 6 weeks now and i have had these symptoms of a upset stomach for about 6 days, dihrrha, weak feeling with no appitite. and i think my lymph nodes in my neck are getting sore.

do you think that i am at risk for getting HIV??? or any std? do you think that i should go get tested for hiv? or maybe these are flu like symptoms. and i just have the flu and my anxity is getting the best of me?

what do you think that i should do???!?!

thank you!!

Welcome to the forum.  Thanks for your question.  I am happy to confirm the brief but accurate replies you had on the community forum.

Oral sex carries little or no risk for HIV, especially from mouth to penis.  According to one analysis, if your partner had HIV, your chance of catching it was somewhere in the range of 1 in 20,000.  That's equivalent to receiving BJs by infected partners once daily for 55 years before transmission might be likely.  In other words, zero for practical purposes.

The risk for other STDs also is very low.  Further, no STD can start to cause symptoms in under 24 hours; 2-3 days is the minimum; your urethral irritation the next morning had to be caused by physical irritation.  (Or soap or disinfectant, if because of STD fear or hygience you tried to use such things after the exposure.)  In any case, if that symptom cleared up -- whether because of the doxycycline or on its own -- then you should have no further worries about it.

Your more recent symptoms are not at all suggestive of HIV or any other STD.  They started much too late to be due to ARS (acute HIV infection); those symptoms always start within 10-20 days after exposure.  As you suggest yourself, your symptoms are most typical for anxiety, nothing else.

But since you're worried, I suggest you have an HIV test.  The negative results probably will be more reassuring than anything I can say.  Enough time has passed for a conclusive result, so don't delay.  If you do it, feel free to return with a follow-up comment to let me know the result.

In the meantime, mellow out and stop worrying.  There is no realistic chance you have HIV.

Regards--  HHH, MD

Re-read my reply above, paragraph 4.  I haven't changed my mind.

You came here for expert, reasonsed, science-based reassurance.  I gave it.  Do your best to accept it.  There is no additional information you can provide that would possibly change my opinion or advice, so don't ask.  If you decide to be tested for reassurance, you may return to tell me the result.  Any other comments will be deleted without reply.

Im sorry to keep bothering you Doctor, but would you say the symptoms that I have now have nothing to do with HIV?? its been 40 days since my exposure.

Current symptoms are
upset stomach
achey sides
a little bit of cough
a little bit of sore lymph's
tingling in legs
have not had a solid stool in a week
a little weight loss, might be from loss of appitite though

once again im sorry, im just stressed out over all this

The oral fluids self test is very good, but may not be conclusive until 3 months.  A blood test at a clinic would be definitely conclusive at this time.
