2nd opinion - oral sex

Hi Doctor, i am male and had oral sex performed on me by a female CSW on 31st July 2011. I ejaculated in her mouth. On the 11th day i tested HIV 1&2 negative through Elisa test. I again re-tested myself for HIV and this time through the HIV Combi Electrochemiluminescense Assay method on the 35th day (5th Sept 2011) and the results were non-reactive (negative).

Since my worry was not totally over, I cross checked with the lab and the machine they use is Roche Cobal e411 which tests for "determination of HIV 1-p24 antigen and antibodies to HIV 1, including Group O and HIV 2 in human serum & plasma" as the report mentioned. This test also checked for Australia Antigen (HBsAg) non reactive; anti hepatitis c virus (negative through Rapid spot test) and HBSAG (non reactive through ECL Assay).

Physically, all through, i feel absolutely normal. Today it is over 2-1/2 months since the 'act'.

Unable to handle the stress I am undergoing psychotherapy and my psychotherapist (Ph.D), a Professional member of ACA, and one who also works with HIV patients also said i am worrying unnecessarily as a 4th gen HIV Combi test post 28 days is absolutely conclusive. She double checked this with a Doctor who works with AIDS patients. The pathologist at the lab confirmed that this (HIV Combi) is a 4th generation test.

I also went to a urologist as i discovered a 5mm calcific density (stone) in the pelvic area through the KUB x-ray while undergoing a battery of other general health tests. I confessed to him about the 'act'. By the way, it is only after he heard about the 'act' that the urologist suggested the HIV Duo test}.

I have read in the net that HIV Combi & HIV Duo & HIV Combo are one & the same thing. I checked with the Pathologist (MBBS, MD) at the lab and he confirmed that HIV Combi & HIV Duo & HIV Combo are same but brand names of different companies (Roche, Abbott, etc).

Please advice if i should go for another HIV test after the end of 3rd month ie in October end.

Welcome to the Forum. I see you are quite worried about this exposure. You should not be. As I see you have already heard, there is no known risk of getting HIV form oral sex- none.  Thus, in most instances, no testing is warranted.  

You chose to test and your tests are negative.  Don't get hung up on the brand of test.  There are two major types of test for HIV- a test for the virus which could be PCR or a p24 antigen test, and tests for antibodies.  The combination of a test for the virus an and antibody test can be considered a “DUO test”.  These tests have many different brand names but all are rigorously studied and tested before they are approved in the U.S. and Western Europe.  When they are approved for use, their results are equivalent.  If you are tested for both the virus and antibody at any time 4 or more weeks after an exposure, the results are conclusive.  Thus, your results are definitive.

You need to understand an believe that you do not have HIV from the exposure you describe.   There is no need for further testing.

This appears to be an unrealted question. Briefly however, Herpes IgM tests are worthless and any result on such a test should be ignored.  As for the IgG, the vast majority of equivocal values are also not inidcative of infection.  I would not worry. EWH

Doctor Sorry to come back but I have a query. Pls help. I was going through my reports & noticed that the HSV 1 & 2 tests have something called as positive, equivocal & negative through Elisa method. I mean each have a value against them. In my case for IgG & IgM both values are in equivocal range. I have again lost sleep, feeling anxious, breathing heavy and full of fear. I have no symbols as I have read in Internet. In recent past an urologist (MS, FRCS) physically examined me and said 'no medical problem' although I didn't show him my STD profile test reports, but showed him the 13 week negative HiV Combi report only. Please help sir.

Thank u for your support. The HIV test at 15 week is negative. You were bang on Doc!! The last 3 months were hell due to 'what if' fears. Now I can sleep in peace. What a lesson learnt. Thank you to all the Doctors on HIV forum and to the staff of this wonderful site.

Doc, only for my peace of mind I have gone for a 15 week HIV test. The result is expected in an hour. I have not had a good night's sleep in weeks due to anxiety & fear of 'what if'. Fingers crossed.

This was a no risk exposure and your test proves you did not get HIV.  I suggest you relax an enjoy your new bay.  I will have nothing more to say. EWH

Doc, sorry to post again. We had a baby 2 days ago. I want to live & be a responsible dad. Few eruptions like boils have come out in both underarms. My GP (MBBS) gave 'T-Bact' Mupirocin cream USP & last time for similar boils he had given 'Fucibet' Fusidic Acid & betamethasone valerate cream.

The sting on my penis has reduced considerably but not gone completely after drinking barley water & cranberry juice. The underarm boils are a new eruption & have brought back the worst fears of HIV.

I cannot confide to my family GP about my oral incident & my psychotherapist is also on vacation.

You have reassured me & i trust you, yet once again I am looking at you for reassurance as Indian doctors ALWAYS INSIST for a 3 month HIV test - i hope my negative 35 day HIV COMBI test (antigen & antibody) is a guarantee of being safe.

Have there been instances of a 5 week (35 days) HIV COMBI negative test result change? How correct is this result especially in light of an oral incident?

I am regretting my oral (getting sucked) incident so much that i never once could fully enjoy my pre fatherhood days & now I am praying that I am safe to take care of my family & never be irresponsible again.

Yoiu do not need testing.  Your tests are to be believed.  Sometimes after an exposure that, in retrospect, one wishes they had not had, persons tend to examine themselves and be far more attuned to genital sensations than in periods when they are not concerned.  This in turn leads to noticing what turn out to be normal sensations that might have been not noticed or ignored at other times.  Perhaps this is a contributor to your situation.  

There is no need to wait for a 90 day test result. these doctors are being overly conservative.  EWH

Dear Dr. Hook,
Sorry to bother you and i promise this is my last post. I hope you will take some time to reply. As i mentioned earlier my 35 day HIV Combi test came negative. Thereafter syphilis, hsv1 & hsv2 and Chlamydia also came negative. However, there is a slight sting at the opening of my penis (the mouth area) and this happens maybe 3 to 4 times in a day. These times i get extremely worried and nervous. Are you certain that i will not have HIV? Do i need a test? I am very scared to re-test. (In India, which is advanced medically, every doctor insists on taking a 90 day HIV test after oral.) I have also not had sex with my wife so far as i don't want to pass on any infection. Please help. Thank you so much.

Thanks Doctor. As a matter of abundant precaution apart from all other tests, i also tested for VDRL & HSV 1&2. Both are negative.

The results of Chlamydia are expected in 2 days. I have extensively searched the internet and i have no symptoms of Chlamydia or Gonorrhea. There is a slight sting on the penis opening/mouth which is similar to a when one gets very high pressure to pee (urinate). There is no burning sensation while urinating and i have checked this every single time. I truly appreciate your help.

I am eating well. There is no weight loss. There are no symptoms of HIV or STD. Your assurance of a negative HIV COMBI test at 35 days is extremely reassuring even when the lab report is signed by a Pathologist Doctor (MBBS, MD-Pathology). Just that i am seriously anxious and very worried about not catching any infection and not passing that to my wife.
