Am I getting infected by hiv? Worried to die...

I am being very scared of what happened on 11/23. I was initially using a condom with CSW (whose HIV status unknown) in China, But somehow during the sex the condom got broken and the head of my penis got exposed. I stopped and checked it out right away it seemed no vaginal fluid or blood on it but i am not sure. After that the girl applied a new one to my penis and i finished it with handjob. Now I am getting really worried the condom broken maybe because her vagina was too dry and maybe i was in her vagina around or less 10 secs...could you please tell me if i am highly risky or what my risk to be infected by hiv? Appreciate a lot...

This will be my last aqnswer.  You do not have HIV or syphilis.  You do not need further testing.  You do however need to forget about this and move forward.  EWH

Dear Doc,

I had an HIV test 43 days after the incident. And the result was negative. Can it be conclusive? Or I need to get another test three months later from the day that the exposure happened? I'm still freaking out, please kindly help me.

PS. I also did the RPR test for syphilis at the same time and the result was negative also. Should I do another one again later?

Doc, sorry. I was thinking what if i didn't remember very correctly and told you about the scheme I told you in the beginning of this post. Just want to doublec check with you again, if I was exposed my own penis in that CSW's vaginal for one minute, cuz i couldn't remember if it's 10 secs or 1 minute at all...Will my risk become very high and you would change your previous opinion? Please tell me the truth...I regret so much...sigh...it's the punishment from God...

Sorry to bother you again, I'm just getting more scared everytime I thought of the incident...

The thing that is huritng you is your anxiety, not HIV.  The minocin will not effect the outcome of your test, which will be negative.  All of your symptoms are likely due to your anxiwty.  EWH

Sorry, doc. I am really worried to die...Lately, I feel something wrong with my penis, so kind of irritation, but not severe...So, I started taking Wyeth Monocin, and I am so worried this might be affecting the HIV test result you told me to do after 4 weeks...But I just can't help thinking if I am truly HIV free as you commented...Is it possible that I might be infected with STDs? I feel slight pain on my penis from time to time...I am really sad now...

There's no reason to hate yourself. You had sex--a perfectly normal human activity/need--and used a condom. That the condom broke is not your fault. You were being reasonable and responsible, and you corrected the situation as soon as you were aware. It doesn't really matter whether you were exposed to her vagina for 10 seconds or 10 minutes. Either way it was pretty low risk.

I think you're spending way too much time thinking and worrying about this. You're making yourself sick with worry for no reason at all. The diarrhea and nausea are likely due to your anxiety, that's all.

My advice, not that you asked: stay off the internet, stay busy and involved in things you enjoy as much as possible, and get tested at 4 weeks as Dr. Hook advised. If you can't handle the anxiety in the meantime, see your doctor and consider counseling.

Sorry, I am still so freaking out, and worried that i might be the exception. I don't know what to do now, if i got infected with hiv...how can i live...i don't know if the scheme i told you was one percent correct or not? What if i was exposed around 1 min, I just didn't know anymore...I am so scared and I hate myself so much now...I have a diarrhea and sometimes i feel like to puke...

Repeating your question does not get in answered more quickly.  Please be patient.

I do not know the source of what you have heard and as policy we do not debate information obtained from other sources - there is just too much misinformation out there to do this.  My answers are based on scientific studies performed all over the world. Some people do get HIV on their 1rst exposure to infected partners but they are very much in the minority. It is a numbers game.  Some people win the lottery the first time they play it too.

Having had chlamydia in the past and having been treated does not change your risk for infection.  Having herpes does slightly increase risk when compared to persons who have never had herpes but in the overall scheme of things the increase is modest.  That you have herpes does not, in any way, change my opinion.  EWH

I forgot to mention to you if I ever had chlamydia and cured as well as HSV II positive but never outbreak or symptoms on my genital part. Is it possible the risk will be so much higher? Also I was heard a lot of asians got infected with hiv by only one time unprotected vaginal sex...How? I am really freaking out...Sorry...Really freaking out that I might be one of them...

This was not a statistically high risk exposure. let me tell you why.

1.  It is not likely that your partner had HIV.  Although CSWs have higher rates of HIV than the general population.  The chances that she had HIV are probably less than 10%, at most.
2.  Even if she was infected, only 1 out every 1000 exposures leads to transmission of infection, on average and given how short your exposure was, your risk is lower than average.

Just on the basis of these statistics alone, mathematically, your chances of having gotten HIV from the exposure you describe are less (and probably far less) than 1 in 10,000.  

I urge you not to worry.  I realize that my assurances only go so far however and thus, I understand that you may choose to also get tested.  If you do, wait for 4 weeks to get tested.  At that time your test results would show infection if acquired 4 weeks before in 85-90% of persons who caught the infection and will be further reassurance that you did not get HIV.

In the meantime, I also urge you do not to worry about every little ache and pain that you have between now and then.  We all have them and they are not a reflection of a recently acquired HIV infection.  Take care.  EWH
