Condom Broke - Help Please Dr. Hansfield

Hi Dr. Hansfield,

I have read a few of you posts and really respect the opinions and knowledge you have passed on to people concerned about the possibility of HIV infection such as myself.

I am hoping you may be able to give me some statistics / peace of mind on the the chances that I have caught HIV from a condom break when sleeping with a Bar Girl  in Patong Thailand. I was extremally drunk yet I wore a condom at the conmencement of vaginal sexual intercourse that went for about 5 minutes. I don't know when the condom broke nor how much of my penis was uncovered and in contact with the inside of the Girls vagina - all i noticed was that it was almost completely torn off when I reached down after slipping out. Once I noticed it, I immediately washed my penis in a warm shower and did not resume intercourse again with the Bar Girl. The follwoing day after a sleepless night, I examined my penis and there were no cuts or abrasions at all. I am now at the 2 week mark and have no symtoms of any other STDs at all. Dr.  I am really scared at the moment, could you please answer the following for me...

a) What the chances are that HIV infection could have occured given Thailand has a high level of HIV infected persons. Do I really have anything to worry about?

b) Have you personally seen any cases in the past where someone has contracted HIV from a condom breakage in Thailand the first time they did something this stupid?

c) What tests would you reccomend? This only happened 2 weeks ago but I'm finding it hard to get the matter  off my mind - I'm really really worried.

Thanks

Hey mate,

I did all most the same thing as you but in Dubai. I am an Australian as well. I have contacted the Melbourne sexual health clinic find them on google they have a great webpage. and they will clear you of HIV after 6 weeks.
Good luck

Welcome to our Forum.  Dr. Handsfield and I share the forum.  You got me.  FYI, the reason we share the forum is because we have worked together for nearly 30 years and while our verbiage styles vary, we have never disagreed on management strategies or advice to clients.

The odds are in your favor.  While the rate of HIV is relatively high in Thailand, most commercial sex workers still do not have HIV.  Further, the risk of getting HIV in the unlikely circumstance that your partner had HIV is low was well- HIV is transmitted, on average, only once in every 1000-2000 acts of sexual intercourse.  thus the odds are very much in your favor.

I find "have you ever" questions a bit silly.  No, as it happens I have not seen someone who acquired HIV following a single exposure in Thailand.  But you can be certain that there are persons who have acquired HIV following a single episode of intercourse but you can also be sure that it is a rare event.  Were this not the case, I would not have provided you with the statistics I mentioned above.

At two weeks a standard HIV antibody test will detect over 50% of recently acquired infection and a combined HIV p24 antigen/HIV antibody ("DUO") test will detect more, closer to 80%.  At 4 weeks the DUO test results are definitive and standard antibody tests detect over 90% of infection.  At 8 weeks standard antibody test results will be definitive.

I hope this information is helpful. The odds are in your favor.  EWH

Hi Dr. Hook,

Thankyou very much for this information. Most definately helpful and re-assuring. Realize "have you ever" questions are a bit silly. I guess one just wants to know of similar experiences etc to feel a little more "not the only one" and that "it will be OK".

I'll play the waitng game til the 8 week mark for HIV antibody testing as per your advice. I'm finding it hard to track down a clinic that does DUO testing in Australia. Would rather not think about it too much in the mean time (if I can get it off my mind that is).

I'll fill you in on the results - will hopefully be informative to the forum.

Thanks again
