You had a low risk exposure. The large majority of sex workers in Puerto Rico do not have HIV; and when a woman has HIV, the average chance of transmission from a single episode of unprotected vaginal sex is around 1 in 2,000. That's equivalent to having unprotected sex with infected women once daily for 5 years before infection would be likely. (Probably you are unaware that many spouses of HIV infected people never catch HIV themselves, even with regular unprotected sex.)
Second, your symptom do not even hit at HIV, which does not cause abdominal pain, tingling sensations, etc as the only symptoms. And it certainly doesn't cause kidney stones.
Third, a negative HIV antibody test at 4 weeks is strong evidence against having HIV. Not perfect, but highly reassuring; over 90% of infected people have positive results by then.
In order to get beyond your somewhat irrational fear, you need to have another HIV test. I have no patience with delayed testing for fear of a positive result. It isn't the test result that gives someone HIV. You have it or you don't, and if you do, you have to know it in order to get life-saving treatment. In addition, anxiety usually declines after testing, even with a positive result. The anxiety and worry are worse than having a clear answer.
So my recommendation is that you have an HIV test and then return with a follow-up comment to report the result. It definitely will be negative. But no other follow-up comments, please. I will have nothing more to say until you report the test result.
