I am a heterosexual male, circumcised and HSV-2+ (confirmed with blood test - Igg 2.34 & I’m assuming it’s genital but have never identified any heptic lesions).

I received unprotected oral sex for approx. 20 min. from a CSW I met off a website (she was white and in her early 30s). The oral sex was vigorous & after there were some small abrasions/cuts on my penis which did not bleed but were clearly visible and stung when I took a shower (these cuts fully healed about 4 days later). On multiple instances, both that night and the next day, the CSW confirmed to me that she did not have any STDs and I specifically asked her about HIV status and she confidently replied that she was recently tested 2 months prior and was negative. I have read on this forum that you advise people will generally tell the truth about their HIV status when asked directly; however, what is concerning me is that during our time together she wanted to have vaginal sex and she said I did not have to wear a condom (although she brought condoms with her) and told me not to worry because she was “clean” (we did not have sex, only discussed it). So, if she really was STD/HIV fee then wouldn’t she want me to wear a condom to protect herself…? I have tried to read as much as possible on this forum & think I’ve learned that the chance of transmission from this type of exposure is very low, approx 1/10,000, and the chance she had HIV is probably low, approx. 1/1,000 (assuming she is not an IV drug user, which I do not think but obviously cannot be sure) so that overall there is very slim chance this resulted in HIV transmission (right?). Unfortunately, like many of the people on this site, an irrational fear of HIV has begun to take hold of me and I cannot shake it off and I’m considering a PCR RNA test which I hope will be negative and will quell my nerves.

My questions:

1.) Am I missing something in my analysis and is this fear of HIV from this exposure really irrational?  

2.) I have read there is already an increased risk of false positives with the PCR test, do you think the risk of false positives with the PCR is even higher given my HSV-2 infection?

Yes PCR has increased positives and are not recommended for diagnosis in cases such as yours. You had no exposure if that was the only incident in question. ONly a 3 month antibody test can clear you ..FYI.  DOn't get a PCR test. Its not worth it. and not conclusive.

Oral sex is not a risk of HIV transmission.
