Elite Controller and HIV rapid test.


I was going through this study and found out that a person who is HIV + had false negative results for one year. He was considered as an elite controller . But my understanding was elite controller are also detectable on Rapid test.

Secondly is there a way to identify elite controller e.g strong immune system and they do not get sick much before or after infection.

It doesn't not tell us if they are immunospressed previously or give any details. I'm not buying this. And also, this was not a well controlled study. The person you're questioning could've put themselves at risk more than once.

It's been 6 years for you. You really don't need to be having these irrational thoughts.

2/3 study visit was on 6 months and 1 year from enrollment.

Hi James, I am glad I talked to you. I am much relaxed now. However, the case for this person is interesting. You see any scientific explanation for this? The strange thing his 4th generation lab test is negative too?

"In this case, the OraQuick test was nonreactive at all 3 study visits (both at study sites and at the centralized laboratory). The UniGold HIV rapid test and the fourth-generation Combo CMIA were also nonreactive at all 3 visits. The INSTI rapid test had a weak reactive result at enrollment, but was nonreactive at both follow-up visits. In contrast, the third-generation Vitros EIA was reactive and the Western blot was positive at all 3 study visits. At all 3 visits, HIV RNA was undetectable using both the ultrasensitive Roche viral load assay and the APTIMA HIV RNA qualitative assay, which is cleared by the FDA for diagnosis of HIV infection. No ARV drugs were detected in the samples, indicating that the participant was most likely an elite controller who may have been unaware of his HIV status."

Yes. You don't have HIV. There's a 0% chance you have it. Simple as that.

James, Thanks that make much sense now. Just a question from your question. Is it not level of antibodies that is detectable by test?

If your HIV level is low (elite controller) , still your body is producing HIV antibodies in less amount though but they will piled up to make a number which is detectable. Am I correct in my assumption?

Elite controllers will test negative on the PCR RNA test. If they continuously tested negative, especially for 6 years long you've been testing for. The fact that there are no antibodies means that they had untreatable AIDS and would be on their death bed covered in sores

Thanks James for reassurance. I know this is busy forum. If possible can you put ONLY few word why do you think i am not one of case given in the study.

Thanks James for listening me.

I had many exposure MSM unprotected anal sex and last exposure was in July 2009.

I had been tested with rapid blood test twice this year(April 2015 and June 2015) and tested twice with rapid blood test last year (March 2014, and June 2014) . All of them were negative.

Thanks James, I noticed you study to become ID. You have any idea why the Case 1 in this study had always false negative? and how do i know that i am not elite controller?

Elite controllers tests negative on PCR tests, not rapid tests
