Going crazy

Hi Doc,
I am a heteroxexual 22 year old male.  In June I was in Las Vegas and got a hooker (one of the skeezy kinds that hangs out around the casino).  I had protected vaginal intercourse with her hopefully the condom didnt brake or fall off (i was very drunk).   It was a long exposure (I would say 45 mins) and i was not able to ejaculate.  Also since I have been driving myself crazy and thought of everything that could go wrong, i had intercourse doggy-style (now I am thinking what happens if I was so drunk and it turned into anal).  I had taken an HIV test two weeks after and it came out negative.  I know symptoms are not an indicator but I had flu-like symptoms 5 days after the exposure, had white sores in my mouth and i have had difficulty swallowing since I have came back (I am pretty sure this is from anxiety).  Lately i have been going crazy (mainly because i had to intern at an HIV clinic for a month for school) and noticed my skin has been extremely dry, my eyes have been dry and i have pimple type things on my back and a couple on my arm my back has a little area of peeling skin (i might just be examining myself too much).  One symptom that is driving me crazy though is ever since i came back from Vegas i noticed after i masturbate my sperm looks different than it looked before Vegas (I can't really describe it that good).  I have no discharge and the only STD i have been tested for was chlamydia due to fear of the swab.  I am going to get tested again this week and hopefully have a peace of mind due to it being out of the window period and eventually get tested for STDs (not only for the hooker just because I never got tested before).  I would just like your expert opinion and hopefully a reassuring one before I go get tested.  Also do you have any clue why my semen would possibly look different (does that have anything to do with HIV)?

Thank you doctor for your fast and reassuring reply.  I agree with you I think it is my guilt and anxiety that is getting the best of me.
I do not want to bother you again but if I understand you correctly HIV nor an STD would alter the appearance of ejaculate?

From what you tell me, it is most unlikely that you have HIV. I think you know that too but the combination of guilt and perhaps anxiety from your exposure is getting the best of you.  It also sounds to me as though it would be best for you to just go on and get tested rather than delaying;  delaying will just make your anxiety worse and it appears to me that at the moment you are wondering if normal day to day variation in you and your body could be HIV.  As you point out, it is not.

AS for the possible change in appearance of your ejaculate, I have no idea.  Your doctor can order a semen analysis to check out if all is OK but once again, my guess is that you are just paying closer attention than you used to.  There are not STDs which notably change the nature of a person's ejaculate.
