HELLO DR HHH

I AM IN CENTRAL AMERICA AND WENT TO A CABARET OVER HERE ON 2 DIFFERENT DAYS. I HAD SEX WITH A SEX WORKERS 2 DAYS STRAIGHT. I USED A CONDOM BOTH TIMES. ONE OF THEM GAVE ME UNPROTECTED ORAL. ONE SEX WORKER PUT SOME LOTION ON THE CONDOM THAT WAS BABY LOTION THAT I UNDERSTAND THAT IT CAN WEAKEN THE CONDOM BUT IT WAS INTACT AFTER WE WERE DONE. I DONT KNOW THE QUALITY OF CONDOMS HERE, BUT THEY MADE SURE IT WAS ON ME CORRECTLY. FROM THESE 2 EXPOSURES WHAT ARE MY REAL RISK FOR HIV....

Welcome to the Forum.  Dr. Handsfield and I share the forum.  You got me.  FYI, the reason we share the forum is because we have worked together for nearly 30 years and while our verbiage styles vary, we have never disagreed on management strategies or advice to clients.

You are not at risk for HIV from the exposures you describe.  When condoms are used properly and stay intact they are protective for HIV and most other STDs. The problem with baby lotion and other products which have oils in them is that the oils may weaken the condoms, as you point out.  A weakened condom is them more likely to break than one which has not been in contact with lotions or oils.  In your case, although a lotions was used, the condom did not break, therefore you were not at risk.

You have no reason to worry and do not need testing for the exposures you describe.  Take care.,  EWH

Purell contains alcohol and repeated use can dry the skim making it more likely to dry and crack.  Occasional use may be OK but there have actually been studies which show that there is no benefit to use of antibacterial substances following sex for the purpose of preventing HIV.  EWH
