Dear dr
October second(19 days back) I happened to visit a sex worker and asked her to do the sex with her hand , she had shaked my penie , I didn’t touch her body at all, suddenly I realized that small wound in my penie and asked her to stop , she suddenly put condum into my penie and paste something in her vagina and start vaginal sex without my permission I suddenly pushed her off and she removed the condom, it was in proper place then I went  to wash room and washed my penie with a soap there in the wash room
17 october after  two weeks I was experiencing throat pain and low grade fever I went to the medical store bought medicine and antibiotic, after two days night temperature  had reached 100 but after that there is no temp. Throat pain is there , now feeling bitter taste in my mouth.
After this incident I couldn’t get sleep ,my concerns she had shaked my penie ,with her hand she used to touch her vagina . will it cause hiv infection
Second thing I washed my penie with the soap there in the wash room all the sex workers and customers using the same soap will it cause hiv infection
Third thing my hair might have touched her vagina and that can touch my wound
I am totally collapsed after this symtoms .
Pls advise wat shall I do

The additional information doesn't change my opinion or advice.  Almost certainly you do not have HIV.  Follow the advice of your personal doctor or clinic.  I won't have anything more to say.

You describe a low risk exposure because 1)  most commercial sex workers in the US and other industrialized countries don't have HIV; 2) hand-genital contact never transmits HIV; 3) there is no risk from condom-protected sex; and 4) the exposure of your penis to her vagina, before the condom was in place, was very brief.  The presence of a "small wound" of your penis theoretically increases the risk, especially if you have genital herpes (more about that below).  But even so, these events carried almost zero risk of catching HIV.  Washing with soap after sex might further reduce the risk, but only a little; washing after sex probably doesn't very make much difference.  It makes no difference if other people used the same soap, because soap instantly kills HIV.

That said, you don't say where you are -- and some of your words suggest English is not your first language.  In certain countries, the chance a CSW has HIV is higher than in the US.  Where did these events take place?

Your symptoms make no difference.  As discussed many times on this forum, symptoms never help judge whether or not someone has a new HIV infection.  It is more likely that you just had a normal respiratory virus.  Also, acute HIV infection usually causes fever that lasts 10-20 days, not only 1-2 days.

The most common cause of a "wound" of the penis is genital herpes, especially if you have had similar wounds before.  When you see a doctor or clinic for HIV testing, you should discuss this with the provider who examines you.  As I said above, having herpes increases the chance of HIV if exposed -- but still the chance of transmission is low during any single episode of sex.  And as I said, most of the time you used a condom.

All things considered, it it unlikely you caught HIV.  But to be completely safe, see a doctor or clinic and have an HIV test a few weeks after the exposure.  Follow that provider's advice about exactly when to be tested.  You can expect a negative result.

Good luck--  HHH, MD
