Condom Split While Having Sex With CSW

Hi,

I am an uncircumcised male, 24 years of age. I had sex(vaginal intercourse) with a sex worker(a girl in her early twenties) in Kolkata(India) twice on 14th Feb, 2015. The first time it was all good.

The second time after I inserted her, 1-2 minutes into the act I was having some different kind of feeling. So I pulled out my penis and found that the condom was broken(I was inside her with the broken condom for not more than a minute). None of us had ejaculated, but yes there was some pre-cum. After this we changed the condom and completed the act. Her vagina was very tight, I was having some pain while inserting her. There was no cut visible in my penis but when I came back home I saw that the head of my penis was inflamed(it was deep red in color). The inflammation disappeared the very next morning though. It has been three weeks now I have no symptoms except on the 10th day some blisters appeared on my penis and it went away the same day.

1. What are the chances that I might have contracted HIV?

2. Does HIV transmit thorough pre-cum? I am worried that the inflammation in my penis might have come in touch with her pre-cum.

3. How long should I wait before getting tested to get a conclusive test result? Also I have taken a H1N1 flu vaccine in Dec, 2014. Will it have any effect on the test results?

This is to update you that I have sent a mail to Dr. Lal Path Lab's customer care and have asked them about the test and it's window period. They didn't give a perfect answer about the window period of this specific test, instead they shared the NACO testing guideline. This is the reply that I got:

As per query received, kindly find the below mentioned test details.

It is a 4th generation test since it includes the detection of P24 Ag along with HIV 1 & 2 antibodies.

We are using Abbott Architect system for S010 test. Test kits used are same pan India.

Post exposure testing protocol includes the following :-
It is advisable to get tested on 0th day of exposure to know the current status after exposure and then get the test repeated after 6 weeks, 12 weeks and 6 months to see seroconversion if any. These are NACO guidelines for post exposure testing.

No worries. You dont have anything.  Sore throat is very common.  Your 7 week test is good results.  Your result will not change.

Thanks for the comforting words.

I was a bit relaxed after the 7 weeks negative result but now at the 9th week post exposure I am again having some symptoms like sore throat and mouth ulcers... I am freaking out again now! :(

I have read in this forum that a "HIV 1/2 antibody and P24 antigen" test is conclusive after 4 weeks(I have taken the test at 7 weeks). Is it conclusive for both HIV-1 and HIV-2. I have also read that HIV-2 antibodies takes time to form. Could someone knowledgeable please throw some light on this.

I took the test at 7 weeks post exposure. So, I wanted to know if it's a 4th gen test.

BTW, have you seen someone with test negative at 7 weeks and then test positive at 12 weeks?

Any generation test is conclusive at 3 months.

The Lab assistant suggested me otherwise, he told me that as far as he knows it's conclusive in 3months time.

Did you ask if this test is a 4th generation test?

I have tried and they do not disclose the name of the testing kit used and all they can say is 6month is conclusive.

I don't know, buddy. I called up the lab from which I got tested, the assistant told me that as far as he knows it's a 4th generation test and the method used is CMIA.

You can try calling up the Lal Path Lab national helpline (011 3988 5050). Please make a note of all your doubts about the test and give them a call. Please also update me about what they tell you about the test, it will also be helpful for other like us.

I know dr lal path as they send all ur blood tests to delho via air...and it is good reputed lab having its national lab in delhi, rohini. Bt i always wanted to know thay which test kit they use for hiv p24 ag/ab tests..is it abott or something else..

I have called up the lab and the lab assistant told me that it's a 4th generation test that looks for both HIV 1/2 antibodies and P24 antigens. He also informed me that the assay used is Carbonylmetalloimmunoassay (CMIA).

It was a blood drawn test, they collected my blood sample yesterday and gave me back the results today.

Does that mean I don't need any more testing and should consider this test as conclusive?

Looks like you are in the clear .
You took a combo test, this means it was 4th generation. All combo tests are 4th gen . Tests from generations 1-3 were all antibody-only tests.

Many nations take a negative combo test result to be conclusive at 6 weeks  (UK, France, Australia, etc).

Your 7 week test is 90% conclusive. Majority people with hiv develop antibodies by then. You will have to take another test at 3 months to confirm. Although I strongly suggest a 4th gen test, which is conclusive after 28 days, so you can stop worrying and move on.

Hi All,

I have just got back my report. The name of the test is "HIV 1/2 & P24 Combo test". I have taken the test from Dr. Lal Path Lab in India and I don't know which generation the test is. I took the test 51 days post exposure(i.e, 7 weeks 2 days post exposure).

The result of the test is Non-Reactive with an index value of 0.15. I know 7 weeks is not conclusive and I need to test again at the 3 month point. Still, can someone please let me know how conclusive is the test and can I put my worries to somewhat rest?
