Unprotected Fellatio

Dr I had unprotected fellatio with a sex worker a month ago.. She gave me a blow job for 3 min. She didnt want me to finish inside her mouth so she asked me to let her know and then she finished with a hand job.
I am worried about HIV. I now have a cough, nasal congestion and a little sore throat, I think its my anxiety driving me crazy.
Do you think I should get a HIV test???
Can I have unprotected sex with my wife??

Welcome to the forum.

Oral sex is safe sex with respect to HIV.  Some experts believe HIV is never transmitted from the oral to the penile partner, whereas other cite data from CDC suggesting a transmission risk of 1 in 20,000.  That is equivalent to receiving BJs by infected persons once daily for 55 years before transmission might be expected, and that's only if the oral partner has HIV, and in the US and other industrialized countries, on average fewer than 1% of female sex workers is infected.

In addition, acute HIV infeciton does not cause cough or nasal congestion.  You have a cold, that's all.

Finally, should you have an HIV test?  On the basis of risk assessment or your symptoms, the answer is definitely no. However, most people in your circumstance still should be tested for its emotional/psychological support.  In general, reassuring words from an expert are not nearly as effective as a negative test result.  As for sex with your wife, of course I cannot guarantee you don't have HIV or some other STD, whether from the oral sex event or elsewhere.  But if I were in your shoes, I would continue unprotected sex with my wife, with absolutely no worries of putting her at risk.

Feel free to let me know the result if you have an HIV test.  You can be sure it will be negative.

Thank you very much Dr Handsfield,  for your quick professional response to my anxiety question, keep up with the good work.
I just wanted to add that my Oral sex exposure was the first time ever, Ive never been with a sex worker before this event.
But there is something Ive been wanting to ask a professional like you for a long time..
If nobody has actually been infected with HIV through Fellatio (receiving oral sex) or there is no real evidence of this, why is there a risk? to me is like  Kissing Thanks again Dr.

Thanks for the thanks.  But I'm not sure I understand what you are asking.  You are exactly correct:  when it comes to HIV, the risk probably is not significantly higher from oral sex than from open-mouth kissing.

Maybe you are asking why oral sex is listed by many agencies as a potential risk factor.  First, there is risk for some STDs, e.g. gonorrhea, herpes (due to HSV-1), and sometimes syphilis -- not as high as for vaginal or anal sex, but some real potential for transmission.  When crafting health education messages, it's often best (and simplest) to not distinguish between individual infections, like HIV versus others.  Second, some health departments or school-based education programs simply are unaware of the facts about oral sex and STD/HIV risk.  Third, when it comes to a fatal STD like HIV/AIDS, institutions often take conservative stands; if there is even a slim possibility of risk, they don't distinguish oral sex from other sexual activities.

So I should really move on with my life.... what is driving me nuts is guilt , but as you say to look rationally to the odds of getting infected with HIV by Fellatio,  is equivalent to receiving BJs by infected persons daily for 55 years, So if I get infected for a once oral encounter I will be making History.....

You also would be the very first person on this forum, in the 7 years since it started, to have caught HIV from an exposure that worried them.  You won't be.

All we can do on this forum is give facts and scientific judgment.  We are not in charge of the guilt or anxiety part.  That will have to end this thread.  Try to move on.
