Welcome to the Forum. I will try to clarify your risks. From the tone of your post, I presume most of the exposures you have described are with commercial sex workers (CSWs). I will frame my responses in that way and, if I'm in correct in my assumptions, please then consider all that I say to be at least a 10-fold over estimate of your risk.
Risk for HIV is determined by whether or not the person you have sex with has HIV or not, how many exposures you have had, sites of exposure and whether or not condoms are used. In your case, my summary assessment is that your risk is low. Here is why:
1. Most commercial sex workers do not have HIV. There is geographic variation in how common HIV is among CSWs but again, most do not have the i infection, world-wide.
2. Even if your sex partner has infection, a single sex act is very unlikely to transmit HIV. There is virtually no risk for HIV acquisition from unprotected oral sex and the risk, if your partner is infected, for vaginal sex is only 1 infection per 1000 exposures.
3. Condoms are very, very effective for preventing HIV if used corrected and they do not fail, reducing risk for HIV by 99.9% over and above the figures I provided above.
With this as background, there was virtually no risk for HIV from your oral or condom protected exposures. This leaves only the episode in Feb 08 when your condom failed and yo have had HIV tests which are negative since then. Thus to summarize, the chance that you have HIV form the exposures you describe is virtually zero. The insurance-related HIV test you have soon will be negative.
As far as your symptoms, the symptoms you describe, including lymph node swelling are very non-specific and in your case are virtually certainly not due to HIV.
You have nothing to worry about from the exposures you describe. your wife is not in danger. EWH
