condom slippage

Hi guys,
I met a danish white young girl who just turned 19 years old, in a matching online app in Denmark and we ended up having vaginal intercourse 3 times in one night. The problem is that on the third time (which lasted around 15min) when I pulled out, the condom was not there anymore. We checked on the mattress and later on she found it inside her vagina and there was sperm in the condom reservoir.  I assume that for the first 12-13min we had protected intercourse since she found the condom with sperm in the reservoir. After that(shame on me for not pulling out straight away after ejaculation- but the funny thing is that it was a very soft ejaculation compared to the previous ones . Unfortunately I barely noticed it) we kept having sex for one more min or two and when I pulled out the condom was not in my penis anymore.
Later on she found it inside her vagina with sperm in the reservoir.
She told me she was totally clean, she always gets screened after she has a new partner despite the fact that she always has protected intercourse, that she has  only had 6 partners in her sexual life and that this sort of incident has only happened to her once, when she had her first partner when  she was 15 years old and that last time she had sex was 4 months ago and she got screened for all STds  two months ago and the results came negative for every possible Std
What scares me is -Don´t you think she should have gotten screened one month ago(three months after she had sex with someone, as CDC advises , for a 100% accurate result on HIV status?
This has never happened to me (condom slippage) and I am very concerned. I noticed that I was much more worried than her after we noticed it. She just asked me whether I have ever had sex without a condom (answer was NO) and whether I have ever had sex with a CSW (I said NO too). Apart from it, she seemed pretty confident and said there was no reason for us to worry about this granted we are both telling the truth. I know I am telling the truth. I just dont know about her.

Is the HIV 28 day test reliable???
In terms of chances of contracting, would you say chances are pretty low? I am sorry for all these questions!!!

No I would not concsder PEP but that is left up to you and your Dr.
Yes shorter the period less exposure.

I read previous answers for similar situations from Dr. Handsfield, and he usually does not recommend testing for such situation. I saw his math calculations multiple times(chances 1 in 1000/2000 in vaginal intercourse if the female partner is positive and when the status of partner is unknown we should usually assume that the probability is the rate of the country (in the case of Denmark is 0.2).

According to Dr. Handsfield chances are extremely slim, almost zero for practical purposes. But I will take the 28 day test for my peace of mind.

Does time play a role on the chances of contracting or not? I know there are no studies conducted on this..but Dr. Hook says it is logical to assume that the briefer the exposure is the better the chances of not contracting any Std.

Any views guys? Thank you so much for your precious help!!!!
