testing?

hi doctor,

i had i high risk encounter back in early september.(unprotected insertive anal for about 15 seconds)with a women i had only new for a month.about 2-3 weeks later i got sick fever, sore throat,.i went to the doctor and they gave me antibiodics which worked really well.theysaid  it was walking pnomonia.so that was followed by about a week of night sweats.so i waited until feb 1 which would be almost 5months post exposure.(maybe 19 weeks).i got a full std panel done including hiv at the doctor(not sure what test they used they took 2 tubes of blood and said it was going to the lab and the lab was quest diagnostics)well everything came back negative.i thought i was in the clear.but now i am getting itching on the rectum i have been take prep h because i thought it was hemmroids.i drive and sit alot.the prep h worked great.but now im getting what looks like white bumps on my tongue.my question is is there anyway that these test could be wrong after 19 weeks.have you ever heard of a test turning positive after 5 months?and should i go back and get retested?finally my doctor told me that i have pre diabeties blood sugar 105 after fasting.could these symptoms be realated to this?thank you for your time.

Welcome to the Forum.  Clearly you are concerned as I note that you have repeatedly asked about this exposure on the HIV community site and the Infectious Diseases Community site.  I agree with the advice you have received on the HIV Community site but am sorry to tell you that the information you have received from the Infectious Diseases Community site is incorrect.  HIV antibody tests are definitive at 8 weeks following an exposure and recommendations to test at 3 or even six months are overly conservative and based on old data which do not reflect the performance of current tests.  Such recommendations all too often represent overly conservative advice given by institutions who feel that the "cannot afford" to be wrong, not that there is any evidence that they will be.

Regarding your exposure five months ago, while there is appreciable risk of HIV from taking part in rectal intercourse with about a 1 in 100 chance of infection IF your partner is infected, the fact is that few women have HIV and even if she was, the risk of infection is about 1 infection per 100 acts of rectal intercourse.

The illnesses and symptoms you describe are quite non-specific and when considered in the context of a negative test for HIV performed as a highly reputable lab such as Quest should not be considered as due to HIV.

If I were you I would not worry that the symptoms you are having are due to HIV and I see no need for additional testing.

I hope my comments and perspectives are helpful to you.  EWH

Your doctor is being overly conservative and, because of that, fueling your anxiety.  The repeat test will be negative.  I'll have nothing more to say on this thread.  EWH
