at what point does high bp start to cause problems with heart and veins,  mine gets a little high like 157/78 than next time I check it , its down in the 139.  so does it to do damage to my heart and artires while its up that short of time, or does it have to stay up days at time to do damage.

plus what does it mean if top number high and bottom number normal like 167/75. what does the top number being high mean like is that from anxiety /stress or does it have a reason. thank you for your answers

If it keeps spiking that high for years, then it is likely to start to cause damage. We all get damage to our arteries, but the body usually repairs them in good time. With high pressure, this task becomes more difficult for the body, taking longer. There is also a lot of conflicting views with heart disease over the causes. First of all saturated fats were blamed, then sugars, then other things. It seems to me that the most logical cause is the very small lipids which are sticky and can fit into the damaged walls of arteries. So perhaps a detailed lipid profile on your blood would help? The tiny sticky lipids do seem to be produced by the Liver when we consume lots of processed sugar.
