is this happening at the same time every month? make sure you keep a log or journal of dates, times and symptoms that may help your dr's pinpoint something.
there were no infections or inflammations found? sounds like what I get when I'm having a really bad episode; I have autonomic nervous system dysfunction; if you're fainting they need to do a tilt table test - sounds like you have some ANS issues going on - mention this to your cardiologist or EP and if you have to go back to the ER

I agree with Jerry, sounds like you need attention right away, if only to rule out that there's nothing serious going on.
If it were me I'd go to ER and get it checked.
