you might experience an alt increase at first which is a good sign because reflects the killing of infected liver cells, let us know how it works epecially on hbsag which is the main target of alinia, hbvdna will lower after hbsag lowering

you might try nitazoxanide which has no resistance and no sides, so if it doesn t work you can stop it safely and will relapse to same situation as now.is your hbeag negative?

if hbvdna is within low range 10-1000iu/ml and hbeag negative you should see hbvdna decreasing by 3 months, the good point is you might also get hbsag negative by 1-2 years, if you have hbsag quantification test available you can see if it is working decreasing hbsag
working dose is 1.5-2g daily, see other posts for alinia or nitazoxanide on how to take it and mild sides it can have, it will be an off label therapy since the drug is used for babies diarrea or immune compromised patients but it is also active for hbv, hcv, flu viruses so you won t find a doctor to prescribe it.it has very mild sides so according to country prescription is not needed, if it is not sold in your country you can order on online canadian pharmacies

unfortunately hbvdna in the serum is not the same as total intraepatic hbvdna so there is no exact correlation with alt/ast.

to have a real measure of hbv activity in the liver a biopsy should be performed with measure of cccdna, antigens and total bvdna in the liver but this can be done only at research level, hopfully we will have this measure by mononuclear cells in the serum but trials about this are not finished yet

in the meanwhile you can only monitor alt/ast and fibroscan to see if there is any liver damage or make a very sensitive hbvdna pcr to monitor also hbvdna in the serum.

the most sensitive commercial assays can detect until about 20iu/ml in 100% cases, between 10-20iu/ml in less cases and between 10-1iu/ml in a very low number of cases, if you can reach a lab at reaserch level the mst sensitive assay is hbv ultraqual:
Mean Detection = 0.62 copies/mL (0.19 IU/mL)
95% Detection Cutoff = 2.89 copies/mL (0.90 IU/mL)
