btw, my viral load dropped to 32 iu/ml 3 years ago, but then it went back up to 1500 iu/ml 3 months later; seems the viral load is not consistant; I think the hbv dna is not distributed evenly in blood.
Is this 3 cm serious ? and why the liver enlarge?

it is a mixture of inflammation mainly and fibrosis very little, usually cirrotic liver does not enalrge anymore and becomes very stiff so your liver is ok
if you mix with fibroscan you can see what is fibrosis and what inflammation, enlarged liver is very common both on hbv/hcv infection and both on fatty liver disease due to poor quality food/diet
check that alt stays under 30 by monthly checks and then if never abnormal by checks every 3 months
check also for hbv precore/bcp mutants because on hbe neg with those mutants (but also without) liver damage is less related to alt values and more to hbvdna values (what happened to me)
since a biopsy is very unpleasant make whatever possible to find a fibroscan so that you can exclude you need therapy and only monitoring is ok
