today i have also read about hcc prevention nucs and interferon and there is an important data:
after hbv clearance there is little residue of cccdna in the cells and hcc can still happen, hcc cells have good amount of cccdna, normal cells don't

so clearing as much cccdn as possible is very important, if etv or tdf keep the same amount of cccdna during the years there is no prevention of hcc even if hbvdna is und.on the contrary interferon lowers cccdna/hbsag anyway even if hbvdna stays detactable

i think nucs and interferon will be the best combo for the future because cccdna lowering is very very important, we should see peginterferon+etv and peginterferon+tdf by 2011

that's the manin reason we are trying it

at the moment the only compounds  (on the market) active on hbsag/cccdna and hbeag are interferon and alinia, from preclinical data it seems alinia is much more potent than interferon and has similar way of action on the host by PKR and Elf2 alpha phosphorylation (not directly on the virus) and has no sides

the other antivirals have very weak impact on cccdna/hbsag and about 10-15 years are required to clear most of cccdna without resistance.the latest data on etv is not good almost no impact on cccdna/hbsag in 2-3 years, worsening of 0,1log hbsag iu/ml on hbe negative

can Alinia also reduce cccdna/hbsag ?
