questions about antibody

i tested negative just after 3 months
but today i've read some articles about the vaccine for Hepatitis B, it is said that only 95% people will produce antibody after 3 shots of vaccination.

i am worried again now...and think about the hiv antibody...

why people did vaccination but can't produce antibody?
is it possible that some people get hiv contracted but completely can not produce hiv antibody?5%?

but i want to ask if the process of "vaccine into human body" is exact the same like some real viruses get into human body.

and are there some reasons why someone vaccined several times but still test negative because of the vaccine it self or the man has some serious immune system problem?
