Patients with HBeAg-positive chronic hepatitis B
ALT greater than 2 times normal or moderate/severe hepatitis on biopsy, and HBV DNA >20,000 IU/ml.These patients should be considered for treatment.
Treatment should be delayed for 3 to 6 months in persons with compensated liver disease to determine if spontaneous HBeAg seroconversion occurs.
Patients with icteric ALT flares should be promptly treated.
Treatment may be initiated with any of the 6 approved antiviral medications, but pegIFN-α, adefovir or entecavir are preferred.
ALT persistently normal or minimally elevated (<2 times normal).These patients generally should not be initiated on treatment.
Liver biopsy may be considered in patients with fluctuating or minimally elevated ALT levels especially in those above 40 years of age.
Treatment may be initiated if there is moderate or severe necroinflammation or significant fibrosis on liver biopsy.
I was detected last july2008.actually my hbv DNA was 10 million only.hbeag reactive.sgpt 64.and took baraclude for 3months as I remember 2008.then I stopped. Its bcoz financial prob.after a year i was test 2009 hbv dna 110million(no medication) up to year 2013 110million(no medication,)yearly monitoring.and last year december 2015.hbv dna increase to 170million.sgpt 200.(no meds)and now this year january 14,2016 I stared taking viread to Feb 14,2016.my test result as mentioned above. So what do you share or explanation about my case. That's the reason why my alt increasing high.bcoz of no meds since 2010-2015.?pls I need ur advise..tnx
