my husband just went to his doctor today for this, he has tried almost everything. he just got the lidacaine today, and has also found that Desitin (baby diaper ointment) helps some. I think it also has alot to do with what fats he eats to take the incevik with. The days he eats really crummy foods (donuts, ice cream, etc) he is worse, when he eats easier to digest fats (flax seed oil 2 tbspns) he does better. thanks for the water soluble fiber tip- i'm going to get that for him now! Oh and I found the lidocaine 5% on Amazon OR behind the pharmacy counter 4% cream at CVS.

I hope you got some relief.

As someone who had excrutiating internal rectal pain (until I figured out how not to) I always answer the posts concerning flaming diarrhea and rectal pain, especially if the person has internal pain. I don't think I could have stood that (crapping shards of glass) pain for 12 weeks. Not even sure I could have stood it for 1 week. And I know it could potentially cause someone to stop treatment, not a good thing when there is a solution to the problem.

Welcome to the forum.

I had both internal and external rectal pain. Internally it felt like I was crapping shards of glass (most likely due to fissures caused by the flaming diarrhea). Externally I had hemorrhoids and burning from the flaming diarrhea. It took me a couple of tries to figure out a solution and once I did, it got better pretty fast, at least the pain and burning did (and no more shards of glass internally).

The key, I found, is to keep the stools quite firm, not hard as a rock, but quite firm. The minuter they get soft (even a little bit), loose, or runny, the pain gets excrutiating.

This is what I did:
I took Loperimide at the first sign of soft or loose stools.
I used Prep. H cream for the hemorrhoids.
I used Prep H suppositories for the internal hemorrhoids and to sooth inside
I used Huggies baby wipes with aloe, as many as needed, each time I went to the BR in order to get every little particle off. Showers or baths work great for this too.
For the external excoriation I used a xylocaine with hydrocortisone (prescription) cream. However, when it got more excoriated, I switched to Desitin, which was qite soothng and healing.

Like I said, once I figured out that it was the soft stools causing it, and I got the stools firmed up, I did not have any more problems.

I hope some of these suggestions help.

BTW, the flaming soft and or loose stools stopped when I finished Incivek.

My husband is on week 8 of tx and had the anal discomfort (in spades) the first week.  Our acupuncturist suggested 15g of water soluble fiber at night, and it resolved his issues within a few days.  He also took 1 immodium for a few days as well to firm up his stools.  I've suggested this to numerous people both here and on his blog, and and received very positive feedback.  You need to make sure that the fiber is water soluble, as I understand that other fibers can bind to the medication.

For me it would come and go. Like other sx - I would never know what other day brings.

Yeah, unfortunately that is a very common side effect with Incivek.  Many people treating with Incivek on this forum have had this side effect.  My husband started having anal rectal pain (burning and felt like shards of glass passing through according to him).  This lasted from about week 3 to about week 13 for him.

Others on the forum have reported that it didn't last as long or didn't include rectal bleeding for them.

In my journal, I have posted some tips that people on the forum have posted over the past few months:  http://www.medhelp.org/user_journals/show/333064/Incivek--Ideas-for-dealing-with-anal-rectal-pain-or-bleeding

The basic idea is to keep the area clean, dry, and protected.  You can try cleaning with baby wipes without alcohol and protecting with Balneol (an over the counter cleanser/moisturizer).  Some people have also said that it has been helpful to keep the stool firm, but not too firm, as diarrhea seems to make it more painful and when it's too firm it hurts as well.  You can try taking one imodium per day.  My husband's doctor also prescribed a lidocaine ointment to help with pain.
