Yes, I had this happen once.
Never had aproblem with the pens,i liked them actually better than the standard syringes.As soon as i screwed on the neddle,the plunger would move and the powder and saline soultion would alwya mix,no problem.Make sure you attach the needle as so it punctures the rubber seal straight on.
It happens. Hopefully, you had another pen standing by and did not delay or miss a dose. Sooner or later you will have a supply gap that you need to deal with. Others have contacted either the pharmacy where it came from (first choice) or the manufacturer directly (second choice). Hang on to the faulty pen in case you need to exchange for a replacement. Don't put it off until your supply runs out.
I have jammed a needle but I thought is was due to not putting the needle on first.
common by all accounts , i dont use redipen but the pre filled syrynges are very cheap too, often not properly filed and honed,meaning they are a bit jagged sometimes
