re: Amerabrit - Adherence/Compliance Papers

Lately I've been thinking about the role of adherence/compliance in medicine taking and wondering just how much it may affect SVR rates. Here are the two papers I found related to adherence/compliance: <a href="http://www.ncbi.nlm.nih.gov/entrez/query.fcgi?cmd=Retrieve&db=pubmed&dopt=Abstract&list_uids=12360468">Adherence to combination therapy enhances sustained response in genotype-1-infected patients with chronic hepatitis C</a> and <a href="http://www.hivandhepatitis.com/2003icr/03_assld/docs/1029/102903_j.html">Age and Adherence Are Significant Predictors of HCV Treatment Success</a>.


The <u>first paper</u> uses data from two seperate trials - with one using non-peg interferon and riba and the other using peg-interferon and riba. They conclude that "<i>HCV-1-infected patients who can be maintained on >80% of their interferon or peginterferon alpha-2b and ribavirin dosage for the duration of treatment in the setting of a clinical trial exhibit enhanced sustained response rates. Our results suggest that adherence will enhance the likelihood of achieving an initial virologic response. </i>"

You know, guy, maybe we CAN be of some help here.  I mean, whatever else we all are...ahem...many of us are researchin' fools, good at ferreting out medical information and making connections.    Maybe folks here can share that burden with you.  

We've done the liver...and now we're going to figure out the lungs.  Onward!

Anyway, think about it.

Thanks so very much for the articles, I am going to read them right away..  I really appreciate your presence here, with or without the links, but you know that already:).  Good to hear you're enjoying All Creatures....  it was once one of my fabourite programs in England; thank goodness for BBC America! My husband and I just rented "Bend it Like Beckham", if you haven't seen that movie you might enjoy it, it's a couple of years old, but will show you another side of English/Indian culture! Characters are very likeable:) Have a great day.

I pray you, your wife and family get through these trying times as quickly as possible. You stay in that "grilling" mode, roast 'em and toast 'em if you need to but keep on top of them.

I am very sorry to hear about your wife's health.  She has a strong support system with you around.  Indiana, I am praying for her and your entire family.   I am sure your children are very frightened and it is difficult to be strong for them, and for your wife, but you are a courages warrier, and I believe, very positive towards challenges placed in your life's path.
I know that you must take time now for them, but please remember, we are here for you.....and anytime you need to "lighten your burdens", please know that we will hold you in the light, and (as I think you might have said once a while ago)

I'm sorry to hear that your wife is ill.  For me, it's always been easier to deal with ME being sick than somebody I love being sick. It's like, "you can mess with me, but DON'T mess with mine".  You've been here since the beginning for most people and your presence will be greatly missed.  All my hope amd prayers your way, Indy.  Take care.

She got sick about a month ago. They said it was pleurisy and sent her home. She quit smoking....after I took her smokes away. I was the "Resident A-Hole" here for awhile then! LOL
Things have been going well....I thought things were fine.... until the other day. She collapsed...right in front of me and the young son(15)!! We freaked out! Called 911. Her pulse and breathing were ok ....but she was OUT. His eyes were BIG! A whole buncha Emt guys showed up and off to the ER we went. She revived and they sent her home with us with NO real explanation. Then....2 days later...it happened again! I am convinced it is blood clots. They have treated her again for this and sent her home again today. I am VERY nervous. I am feverosly(sp) researching this stuff and looking for answers. These docs seem to have none. That is NOT acceptable to me.
I am still availiable through E-mail for any of you. Everybody on this planet has my address. It may take me awhile to answer but I will get there eventually.
Hey Showboat....You can get my address form Berlynn if you don't have it. You can write me anytime. You 2 need each other and are good support for each other now. She is a very trustworthy person and is "safe" for you to deal with. She needs all the help you can give her right now.
I will post if I get any answers on this situation, but it looks like this will be a long term thing.

Hey gang. Things here have been changing rapidly and I find that I will no longer have time to spend on the computer anymore. Wifey has taken a nasty turn for the worse(2 ER trips in the last few days), and I need to devote all my spare time with her and my youngest son who is still here at home. I find myself in my "Grill the Doctor" attitude nowadays so I wouldn't be much support here.
It's kinda weird now as I look back. I remember my son asking me 2 years ago while I was in the depths of TX if I was gonna die. I probably looked like it to him. I cried my eyes out and assured him I wasn't....but inside I really wasn't sure. Now, today he asked me if his Mom was gonna die. I cried again, and assured him she wasn't....although again I'm not all that sure. I really feel for him. So I will spend all my spare time with him and try to take his fear away.
I am Sooo glad I did my Tx when I did instead of waiting. If I had waited then I could never have dealt with all this and may have found myself never getting rid of my Dragon.
Patient compliance with the dosing schedules and Positive Attitude are the 2 things that I believe you all can do to give yourselves the best chance of beating this thing.
I wish you ALL SVR and hope your sides are light and few. I care about each and every one of you...even those who don't like me much. I hope you ALL kick your Dragons A$$. NEVER give up!!

Awwww,,,,Darn!! Just when we have been on a roll lately for good news and this makes me so sad to hear this!  You and your family are in my prayers and if there is anything I can do or just be a sounding board,,,You know where I'm at! As far as the SVR bus,,,I started this trip in beginning knowing,,,Me and Hubby will clear and we are going to be on that bus as SVR's. I have told my hubby all along about the driver,,,Indy!!  So I'm holding out for you but hey,,,,Sammy is a great choice also!! haha I could hang with him!  Take care and please keep us informed because we do care!!  Love you!!

Thank you very much for your kind words.

I'm betting BIG TIME that your body has been thoroughly swept clean of any and all Hep C virons - and SVR is but a mere formality.

I'm also betting that my liver is soon to be as clean as your is. I've left the doldrums of the 20's behind and just did shot #31 a few hours ago - more dead virus being shed from my system as I type this. Can't wait for the next four months to roll by - but I prefer to try and keep an eye on things week-to-week, day-to-day and, when needed - minute-to-minute.

P.S. - don't know if you saw a post I did to you a few days back. I mentioned that my wife and I have been watching episodes of All Creatures Great and Small, and it has me wondering if you have any personal familiarity with the Yorkshire Dales?

I look foward to readig these items as complence/non has been of great interest to me. I stuck to the higest doses available to me and i live with real(as you can imgane)hope that my endurance of both drugs was woth it. The <u>second paper</u> uses data from two trials of genotype 1 patients who used Pegasys and riba. From this they create a model. And within that model they input data from two theoretical patients, one aged 20 the other aged 60. From this they predict "<i>SVR rates of 74.4% vs. 44.2%, respectively</i>". When adherence is factored in, "<i>the probability of achieving an SVR increased to 85.4% and 52.9%, respectively</i>" - a predicted increase related to adherence of 11.0% and 8.7%, respectively.
