I got my 2nd lab results today and liver biopsy report. But the dr. wasnt able to be there.

biopsy  cronic hep with mild activity and no firbrosis, lymphoid follicles and steatosis consistent with hep c.  Liver with retention of the norm. hepatic architecture. mild fatty change is present.  portal traids show focal increased portal inflam. with interface activity (grade 2).trichrome stain reveals no fibrosis. no increase in iron deposition by iron stain. oh and i got a small echogenic lesion in the superior portion of the right lobe of the liver which is suspected to be a hemangioma 1.1 cm. that was not biopsied. I get some of this part.  But  the last blood work I had listed all kinds of readings of different things. All normal but 2 items ( hubby burnt my paper work , should of got a copy today. dumb me) This lab  only says 201721 H <43iu/ml  =hcv rna, pcr,quant   and   hcv rna, pcr, quant 5.30 H <1.63 log iu/ml ...I figure the H stands for high being not good. seems the bigger number of ? and the smaller number of? are higher than my memory of last results. I am sick and alot of stuff has been going on . marriage, car burnt up, things like that. Can any one shed any light since the doctor was out today and I wont get to see him for a month.

The good news is that it appears from the report you are Stage 0 (no fibrosis, which is good, because fibrosis=scarring!).  However, you do have fat infiltration in your liver cells, which is not uncommon with hep c.  The "portal inflammation and interface activity" is pretty typical of active hcv infection (inflammation is the liver's response while fighting the virus).

As far as the blood test results, the 201721 most probably is a measurement of your viral load (VL) at the time of draw. The other number, 5.30 (log) is just another kind of scientific annotation for saying 201721 iu/mL.  The <43 and <1.63 log represents the sensitivity of the particular VL test that was used, that it will tell you if you have more than 43 iu/mL of viron copies, but can't count below that level.  (43 units = 1.63 log)  Anything more than 43 as a VL on that test would be marked with the "H" (high, or out of the expected "normal" range of the test.)  Hope that helps, and welcome to the forum. ~eureka

We'll try to treat you well here on the forum, and we can try make you feel better when you need it, but if nothing else, the doc's handy to have around to do the paperwork (testing, scripts, etc.), and easy to blame if you're not feeling comfortable ;).  You and my husband seem to have similar views on doctors (but unlike you, he doesn't like our cats' vet either lol), but just as with anything else as is with doctors, there's the good, the bad, and the ugly.  

It's too bad that bedside manner isn't a pre-requisite to be a doctor (though it should be!), but with hep, it's definitely important to have a savvy and experienced hepatologist or GI -- sometimes the sacrifice of a soft touch is worth it for hard knowledge -- it's in your best interests to get someone who knows a lot (plus arrogance, I guess), even if he is a ... um... .donkey.  Maybe you can send him to your nice vets to experience some bedside manner ;).  Wishing you luck moving forward.

You guys are better than my doctor...lol...Maybe I should get you guys to treat me. I honestly would feel more comfortable with my 2 good vets or you guys treating me. I have this thing with doctors. I like my regular doc. But I have been like an advocate for others with other ailments. And I eighter like em or I really dont.  Ive even caught em lying. I go with my gut. I have only seen my GI 2 times. My impression is he is a J---. Maybe he is a really great doc though with a lousy beedside manner.

Those numbers are the same thing but only expressed differently.

201, 721 iu/ml is just another way of saying 5.30 in log form and quantifies how many virons are in your blood, that is, your viral load.

Viral load fluctuates and anything under 400, 000 IU/ml is considered low.

Ditto on what Eureka says. It looks like you are in pretty good shape. I know that viral load number looks high to you, but it isn't. We only really worry about that when we are doing treatment because then it shows you how well the drugs are working at killing the virus, and you want it to be all gone by a certain milestone week... for me it was week 12. I think it is different for different genotypes.
