und at week 8 but now..

week 12 after ribavirin reduction it says details <43 lowest 18 iu per ml.   so now what?   I guess I have to do more than 28 weeks?    think it was from the ribavirin reductions?

My husband's viral breakthrough was with a low VL. He was UND and then Det less than 28 or something at week 24. He did triple therapy w Inc and so he had to stop. Just saying, it can be a very low VL and be a breakthrough sometimes. Glad you were able to continue and glad you went UND again. Keep us posted, and praying for SVR for you.

She left me retest @  17 weeks because I wanted to quit. I was und and @24 weeks still und!  Don't lose hope!

she said no to retest.  that w victrelis the important labs are 8 and 24.  that at 12 if vl is under 100 u continue. I was und at week 8 so she said I have to wait til week 24 to figure out what to do.  has anyone else ever known someone that this happened to?  I wonder it I'll have to do 36 weeks now.

Well I agree a retest is needed, there have been a few breakthroughs here and while treating their viral load remained low, we have also seen a few lab errors also....... If is is a breakthrough I would think they would stop treatment.......... This is very important for you to find out ASAP, please don't let them wait until week 24 before doing another viral load.... Wishing you the best, I know this has to be driving you crazy.

I agree with ceanothus that you should get a retest ASAP so that you know for sure if you are DET or if this is a lab error.

Viral breakthroughs can hover around the DET/UND limit while a person is on treatment. Some people on the forum have become UND and have then gone on to become DET and then hover around the DET/UND limit, sometimes going back and forth between DET and UND.

The Victrelis futility rules call for stopping treatment if the HCV RNA is 100 or above at week 12. Even if you are DET, you are below 100.

"Discontinuation of Dosing Based on Treatment Futility"
"Discontinuation of therapy is recommended in all patients with 1) HCV-RNA levels of greater than or equal to 100 IU/mL at TW 12; or 2) confirmed detectable HCV-RNA levels at TW24."

I'm so sorry to hear this. It does sound like an unusual result because, as Willbb said, usually a viral breakthrough results in much higher numbers than this. So it sounds like viral breakthrough but at the same time it doesn't sound right, which I think hints at lab error. The first thing you need to do is ask for (or demand if necessary) a retest of the HCV PCR. Try to hang on to your hopes until the retest results are back. Don't stop tx or anything like that, not unless a new test confirms viral breakthrough. If it is confirmed then you will have to stop tx and wait for the next generation of drugs to be approved, but I hope that is not going to be the case. Best wishes!

it says hcv rna detected <43 iu/ml lowest detection 18 iu/ml and for log it shows <1.63

for my 8 weeks same area says hcv rna not detected log not calculated.  I'm freaking and no one called me back yet

If one is tested UND . and then at anytime during the course of therapy  becomes DET. again this is known as a "viral breakthrough" ,and is qiute rare.
Also,I would mention, if this occurs it would usually be that the viral count is much greatet than this.

My advice is the VL results should be verified, either by retesting the same specimen if the lab still has it or by getting a redraw soon. If it was me, I would want to be certain that the lab did not make an error when doing the VL.

so my beginning vl was 1.5 mil 4 week lead in 28k 8week und and at My 12 week it says det <43iu/ml

Can you copy and paste the full lab report for interpretation? At what week was this lab done?

What would be important  if the <43 was followed by the designation DET. or UND. on the amount of time  of total therapy recommended.
