Oral or Genital?

Dear Doctors,
I’m a 25 year old male. On 7/7 I resulted positive for HSV1 blood test. What induced me to take the test was the following:
On 3/31 was given unprotected oral sex by a stranger. On 4/24 had unprotected sex and oral sex with another stranger. On 5/30 was on a beach and stayed with wet swim suit for a while and then put on pants without underwear for few hours on the beach. On 6/8 had unprotected sex with my diabetic girlfriend who suffers from yeast infection and a few hours after or the day after started feeling unbearable itching on head of penis and shaft, some white discharge, and discomfort while peeing. Along with it some days later the shaft (inside) became very dry and I got two tiny cracks, I believe from scratching, that burned when was washing with soap and healed after two/three days. I decided to go to dermatologist because after all of this the shaft (inside) became very red, irritated. Therefore dermatologist gave me an antimycotic cream for ten days for he said it was a result of bacteria from the beach whereas candida was not there anymore. Today 7/13 is my 5th day and red didn’t go away but what was left of the initial itching did.
Therefore I’d like to know if any of these can be considered symptoms of a first genital HSV1 outbreak since as far as I can remember I'd had no previous outbreaks apart from recurrent warts, twice a year, near my lips that get burnt by dermatologist and that once I passed to my girlfriend down there.
If I have genital HSV1, can there be any complications for my diabetic girlfriend in normal life and/or pregnancy? What if it’s oral and very weak or asymptomatic, do I have to be careful?
I cannot figure out whether my HSV1 is oral or genital and whether I contracted it during childhood. What’s strange is that what seems to be in my opinion a first genital outbreak happened more than a month after the last 4/24 unprotected sex. Can you confirm that?
Many thanks,
Mark

Hi Mark,
If you were going to develop genital symptoms with a first infection, they would like appear within 2-10 days.  Your symptoms do sound more fungal than herpetic, though the white discharge (was it from the urethra?) is a concern, but not necessarily for herpes.  Has your girlfriend been tested for HSV 1 and HSV 2 antibody?  If not, I would recommend that she do that to determine if her status is the same or different than yours.  Have you ever in your life had a cold sore on your lip?  If yes, then we know your infection is oral, not genital.

Those numbers are weird, are you sure?  The 0.90 makes sense, but the others, not really.  The little sore in your nose is very suspicious of HSV 1, however.  Do you have an actual copy of your blood tests you could review for me?

Dear Terry,
thank you for your kind and immediate response. I used the WRONG term while describing my symptoms. In fact I used DISCHARGE but it was more of a flaky substance on the skin.
I have never had a cold sore on my lip but I sometimes get a tiny wound in my nose but that goes away after usually 3 days. However, I underwent 4 months of chemotherapy about a year ago for a testicular cancer and never had anything and thought that the virus gets "pleasure" out of low immune sistem and considering that my defenses were close to 0 shouldnt have it come out then?

DOUBT On my blood tests there is:
HERPES SIMPLEX = 4.40 NEGATIVE
HERPES 1 VIRUS = 56.00 POSITIVE
HERPES 2 VIRUS  = 0.90  NEGATIVE

Does this make any sense?

Many Thanks,
Mark
