2 positive cultures. 2 negative iGG. Confused

No sexual intercourse with a male since 2003 and last sexual contact with female was in mid Oct 2012, first HSV-2 out break was in May of 2013, 2nd June 2013, way past the date that it should show up in the iGG blood test, but I had a positive culture in May of 2013, again in June, again positive culture and 6-8 weeks later negative iGG blood test.2 positive cultures and 2 negative iGG blood tests..

Another outbreak in Nov and Dec 2013 and now again Jan/Feb 2014, They seem to happen within 6 weeks of each other, Usually I get a cold sore that happens before the HSV-2 but not this last time.

The now infectious disease doctor involved said she in her 28 yrs of practice has not seen a positive culture and not a positive iGG, especially after this long of not sexually activity.

Plus I read this: HSV infection has also been associated with cognitive deficits of bipolar disorder, and Alzheimer's disease, although this is often dependent on the genetics of the infected person.

I have Bi-polar I, and there are questions as if I have MS, because I have most, if not all if the symptoms and a questionable MRI.

Anyone have any ideas?

Maybe it is, but it is best to be sure and remove any element of assumption.

We are dealing with: no sex with a male for a decade, no known partner with HSV2 and no IgG antibodies. What are the odds of HSV2 given these three observations?

Wouldn't this HAVE to be HSV2?  She already has an established oral infection.  And HSV1 wouldn't cause 4 breakouts in 6 months except for in rare circumstances.

I guess she could have transferred her own HSV1 from touching a cold sore and then touching her genitals; but, beyond that, hard to not think this must be a type-2 genital infection.

Note that I am referring to the possibility a person carrying HSV for a while before having a noticeable outbreak or experiencing a change in pattern. People will generally test positive within weeks of infection I agree.

You are describing your blood test result from Herpeselect for HSV2, what is your value?

The same paperwork for the swabs would be invaluable as well.

If you have sores now, request a swab and instead of a culture insist on a PCR which very accurately types for HSV.

Not sure of the cost but taking any of the antivirals will help reduce outbreaks and aide in healing.

ok let me repeat what I think I hear. I am positive for HSV-2 in the genitals,  unknown for HSV-1 in the genitals. I know I have cold sores so I am gonna say I am positive in the HSV-1 on my face.

May not be the state of the art test, but supposedly live in a state of the art "Medical" state DMC... Clinic.

I can see if my insurance will pay for the western blot test. I heard it is not cheap. If it is gold standard why is it not done more often? Cost?

is HSV immune suppressive or immune active? Over active or under active I guess I am trying to figure this out. Is it attacking of system or helping it?

What are the best meds to control the symptoms if you have out breaks a lot and what are the consequences of taking these meds? Anyone?

Fleetwood is correct that a positive swab would be conclusive and its possible that you have hsv1 from childhood. As far as negative blood tests, even the best blood test miss 10% of infections and if your test was not the state of the art might make missing them higher.  At this point you should have the Western Blot blood test as is considered the gold standard in blood testing.

Oh wait I also read this on the paper...HSV type 1 was not testesd because of the following. The incidence of HSV-1 infection in the presence of HSV-2 (dual infection is extremely rare)

I have open sores now, who thinks I should get this tested for Type 1?? thoughts on this?

Guess who gets the most extremely rare everything to everything in the world of medicine? ME, every medicine on the planet. I get the side effects, the rare side effects that doctors are at Aww at. Good Lord. I think the sores need to be tested for HSV-1 also, then what. What happens if they come back positive for HSV-1? How does the testing person tell the difference between the two, is it type 1 or 2. man what a mess of medicine world.

Not according the infectious disease doctor.

You should really test positive in an IgG test with in 3-4 months from the 1st out break and for sure after 1yr from your last sexual encounter and I have not. Only positive in culture. It is not in my blood. I have had cold sores since I was most likely a teenager. I had shingles when I was 16 or 17 on my chest, tested positive and took medication to stop it from moving. Cleared up fast. I have scars though.
Here's  what the Paper work says following: Lab: CB, -HSV 2 IGG, Herpeselect Type Specific AB
Index 1.10 Positive
This assay is type specific and will differentiate between HSV-1 and HSV-2.  A single positive result only indicates previous immunologic exposure and the level of antibody response may not be used to determine active infection or disease stage. The test should be repeated in 4-6 weeks when negative or equivocal results are obtained in suspected early herpes simplex disease.

I had it repeated even though I was negative and it was still negative.

Lost totally and completely lost on my health and I have had 6 out breaks now with in a year.

I am frustrated yes, but I do hold hope, I keep on keepin on. Keep the info coming seems to be a lot out there. Thanks for the insight and any further questions I will be happy to answer,

Check paperwork carefully. But yes, a correctly typed culture is definitive as to having that type of HSV.

How long have you had HSV1? If since your youth then this would ordinarily give you protection against a genital infection, but not complete.

It is possible to carry the virus for a long time and certain shifts in your body and its immune system can result in a changing pattern of outbreaks.

I would have to look at the paper work again but I do believe that they told me they know I already have type 1 so they were not testing me for type one only type 2, that came back negative. but regardless even the specialist said because the culture came back positive for type 2 I have type 2, the person who may have gave it to me, I went and basically took her head off, dragged her to the testing place and she was clean for everything.

So how much since does that make. She's clean and I have not had any sex then since 2003..I am so lost, confused and just....UGh.. Yeah oral sex is my thing and I am not ashamed to say it on here, its life, SEX, So I am thinking that is it most likely type 1, they just haven't figured that out.

Don't know what to do, Specialist said if I have more than 5 outbreaks in a year I need to talk to her about a maintenance drug. and the consequences of being on this, sounds fun, Not!

Any ideas on what other testing I should ask for as I am having an active out break Now?

Many cultures are not properly typed, just tested for HSV 1/2.

It is more probable in your case that you have been infected with HSV1 and it's not showing in commercial blood tests. A Wesfernblot is likely to be conclusive for you although in exceptionally rare cases your body may not manufacture the antibodies being tested for. This is very rare and more typically associated with HSV1.

Are the cultures actually being typed for HSV2 or just HSV and then an assumption made owing to location?
