From what I have read, auto-inoculation is pretty darn close to impossible.
In an answer to your questions:

2. It's usually a cluster of lesions from what I have seen.
3. HSV-1 is a strain of a virus. The only other strain of this herpes virus is HSV-2.
4. That could be a possibility, but protection was used, so the likelihood of that is rather low.
5. Valtrex will not have any effect on an IgG tests. An IgG anti-body is always positive once a person is infected with HSV. However, certain tests for HSV-2 can produce a false positive IgG if you already have HSV-1 in your bloodstream. Most or all of the tests for HSV-1 or HSV-2 can produce a false negative IgG if you are recently infected.
