Urethral herpes? or just from friction?

Let me start out by saying, I'm a bit of a worrier and a hypochondriac when it comes to stds.
I am dating this girl currently and we always have protected sex with a condom, except the one time the condom broke and one time when we were both drunk and didn't have a condom. I heard rumors she had herpes (small town) and confronted her about it and she explained that a girl who wanted to get with her ex-boyfriend a while back spread that around and the rumor got back to her via a friend. She said that she had been tested a year ago and doesn't have anything. I've never seen anything other than razor burn on her, but anxiety gets the best of me. She's going to let me see her test results once her mom gets them to her (she was under 18 when she was tested).

Now for my issue: After a night of drunken rough sex, the next morning I was just looking at my penis and checking all around and noticed on the inside of my urethra (I have corrected hypospadias, so I can fold the skin back a little) there was a tiny red bump. It didn't hurt to touch or burn when I peed. It's just one single bump. When I was in the bath and I inspected it thoroughly and it seems like I could move a piece of skin that was over the bump like when you tear your skin. It's still there 4 days later, but still no pain or other symptoms like itching. I don't have any other bumps or redness anywhere else and the tip of the urethra isn't red or inflamed.

I have previously been tested for everything over a year ago and I didn't have HSV 1 or HSV 2.

Is it possible to tear the inside of your urethra like that? and could this just be an abrasion from rough sex?
She's a nice girl and has told me her sexual history already, so I don't think she'd lie about something like that especially when she's willing to show me her test results.

I just need some reassurance that it's not some "atypical herpes" or something. Thanks in advance.

If you have ever had a cold sore on your lip, the antibody is being generated by THAT infection.  It is highly unlikely you would get HSV1 in a new location on your body following oral infection.

Your HSV 1 test is strongly positive (have you ever had a cold sore?) and your HSV 2 is negative.  This says you are infected with HSV 1 but not HSV 2.

That index was supposed to read

1.10 positive

And the first Immunoglobulin G range is 1198, not 11198

Just a follow up with actual test results:

2/21/2011

That bump is still there unchanged, but I mentioned to my dermatologist today at the follow up appt about having corrected hypospadias and he said it's possible it could just be skin from the surgery. We scheduled an appt in 2 months and he may recommend me to a urologist. I plan to be retested to be safe.

He said as the results go, it's not a currently active infection but do have antibodies for hsv1, but none for hsv2. I guess I kind of expected hsv 2 to just say 0.00, but are these results anything to be concerned about? Is the >5.00 H hsv 1 value abnormal or is that about where a positive hsv1 reading falls?

Yes, I would agree that your HSV 1 infection, wherever it is, is likelyl NOT a new infection if the index value is >5.  However, if you acquired new infection 2-3 weeks ago, you could have this index value now.  If you had a fever blister in the past, I think that is a much more likely and clear answer to why you have HSV 1 antibody..  

Genital HSV 1 does recur and shed less often than HSV 2, yes.

The dermatologist called me today with my lab results and are as follows

IgG & IgM were negative for HSV2, but positive for HSV1. The numerical value he said for hsv1 was greater than 5, which test, he didn't mention.

He did let me know that hsv1 can happen genitally, but he still stands firm by his original diagnose of just an abrasion. He said if it recurs again it could very well be hsv1.

Questions:
1) Since this cut showed up about 11 days ago, and the antibodies were positive for hsv1, does that mean the infection was acquired at least 6-8 weeks ago since it takes around that time for antibodies to develop?

2) If it were hsv1 genitally, is that better than it being hsv2 genitally?


I doubt that it's genitally because I do recall over last summer getting a fever blister (or so i thought) but it went away within a week.

Can a dermatologist properly diagnose a herpes lesion based on physical appearance? I asked him to swab, but he said he didn't need to. I hate to doubt his judgment, but the fact that people say atypical herpes is more common scares me.

What exactly does an atypical lesion mean? and can a dermatologist tell the difference?

He said I'd be in a lot of pain if that was indeed a herpetic lesion in the meatal opening. Is there truth to this statement?

Just did. Dermatologist said it looked nothing more than just irritation. Nothing he would be concerned about, gave me bactosin or however it's called and gave me a script to get tested just to be safe

Herpes usually changes its appearance after 6 days, either improves or gets worse.

Herpes discharge might be clear and minimal.
A tear from rough sex might take a couple of weeks to heal.

Can you describe the discharge? I noticed something earlier, but I couldn't tell if it was anything abnormal or just normal secretions possibly from nocturnal emissions or maybe just urine. It was clear and nothing came out when I "milked" my penis. I'm trying my best to not focus on it, but my anxiety always wins. It hasn't hurt when urinating except maybe a slight sting (minimal).

If this was just a normal tear in the urethra, how long do you think til it heals?

It's day 6 and it has remained unchanged in appearance, just slightly red.
Should the fact that it's unchanged after 6 days be more of a reason not to worry?

An hour of continuous sex could cause some irritation for sure.  Herpes in the urethra might present as pain with urination or perhaps some discharge.  

Checking a lot is common, don't feel badly, but it can create its own set of problems, for sure.

Thanks so much. You're right about looking so closely. My anxiety allows me to create these problems. I have never really folded the skin back to see inside, no. Otherwise, I would've never noticed it.

Do you think rough sex could cause a tear or cut right on the inside close to the tip? The skin there is quite thin, but on the outside it is fine. We had sex for a little over an hour.

How would herpes in the urethra present itself? Would there almost always be some discomfort or pain when urinating?

Ok, enough questions after this I'm sure you're tired of answering all the super anxious follow ups.

I have two theories about STDs that I think are accurate.  If someone comes into my clinic with toilet paper in their underwear to capture discharge, they have gonorrhea.  The second is that if someone has to look either with a magnifying glass or have to look as closely as you describe to see something, its probably not herpes.   In addition, your partner sounds reliable.  In addition to that, many people have little bumps in the urethra that are completely normal.  Have you ever looked this closely before at the inside of your urethra?  
How about you wait and see what her tests show before you worry any more.  
I would like to reassure you that I don't think your symptoms are herpetic.
