looking for solutions

Hello Terri:
will try to keep this somewhat brief. 24 yr old male, heterosexual, only been with 5 woman and now engaged.

2 yrs 10 months ago before I knew my fiance I had unprotected vaginal and oral intercourse with a unknown female at college. i later found out that she has been with 20 men.
~ 2 wks after this I had  NGU and a red rash just under my urethral opening. this progressed into being painful upon ejaculation  and a week later totaling 3 weeks after intercourse I had a mono like illness. ( red tonsils cryptic tonsils, with white spots on them, low grade fever topped about 100.9 , dry cough, conjuctivitis in both eyes, sore throat that seemed to last 4 or 5 months. ( the main illness was about 1-2 weeks.) I have had Swollen lymphnodes on both sides of neck, armpits, and maybe pea sized ones behind my right ear for 3 years now.
I went to the student health clinic and did a urinalysis for chlamydia, gon, and trich all negative at 7 weeks total past intercourse. All neg. I

I get frequent Cold sores on my gums guessing this is HSV1. like once a month ,maybe. I work 45 hrs and school full time while studying for MCAT also. high stress.

Originally urgent care doc thought the red bumps on my glans penis was HSV2 . never blisters or anything just redness. Neg. swab test.

I often still get tingling down my legs could this be HSV2?

I have had 18 negative HIV tests mostly mouth swabs , a few Blood draws, and a Undet. HIV Viral load. I have tried putting HIV out of the question. I tested to 2 years 3 months past this exposure.

1) could all the pea sized lymph nodes be from HSV2?
2) Doctors have said no way this could be from HIV after 2 years of negative tests even though most have been mouth swabs. you thoughts?

3) I had a neg monospot test. Could this be from EBV ? ( I know this is the virus that causes mono but heard EBV can cause symptoms . the mono spot was 7 weeks after this act.

Can you give me your thoughts please.

I disagree that someone can take 36 weeks to seroconvert to HIV.  I honestly think you can stop worrying about that now.  The vast majority of people seroconvert to HIV positive status within 6-8 weeks after infection.  You could look at the posts by Dr. Handsfield on the STD board for confirmation on this.
Anything on in the head or in the skin in the area of the chest could cause lymph nodes to increase in size.

Thank you for your response. Nope I have never had any canker sores on my lips.  and no I have never been tested for HSV 2 other then a swab test on the head of my penis but like I mentioned there were never any vesicles or blisters. Just redness , pain upon ejaculation, ect, then the illness came.

I will get checked for HSV2 as well.

I would also mention that I have cryptogenic tonsils , they are not swollen but have remained extremely red for nearly 3 years now. Every now and then I get tonsil stones due to food in the crypts . I will most likely get my tonsils removed when I can. Could cryptogenic tonsils be causing the generalized swollen nodes?

Yes I know I have been tested several times for HIV. The only reason is because I have had a negative mono spot test ( rapid at 10 weeks past encounter) . negative urinalysis, for chlamydia, gon, trich. negative Hep B surface antibody. Despite all the swollen nodes one behind my ear, one one on both sides of my neck, and under my arms. Since this happened I have also had a pyogenic granuloma on my shoulder ( im 24) which had to be lanced off.

So I always end up reverting back to the possibility of HIV.  It seems that there is so much uncertainty around the " window period" . My fiance is a RN,BSN and I ahve heard about studies which state antibody detection has taken as long as 36 months . This was from her professor. She is graduated now.

Can you please give me your thoughts ?

1) Do you think there is any possiability of this being HIV?

2) besides my tonsils , I also have 2 or 3 pretty bad teeth. I know this could cause things in my neck but I did not think it could cause issues under my arms  or granulomas ect.

3) Your advice and recommendations would be greatly appreciated. i will not ask any additional questions after this.

The sores in your mouth are canker sores, and canker sores are not caused by herpes.  Have you ever in your life had a cold sore on your lip?  

HSV 2 would not cause continuing swollen lymph nodes, especially in parts of your body like arm pits.

You certainly could have acquired some kind of virus from this sexual encounter, but I seriously doubt that it would account for symptoms that you are having 2 years later.

The oral swab tests for HIV are as good as the blood antibody tests.  Eighteen tests is two years suggests that you might be a little obsessed with this possibility, and if you were my patient, I would refuse to test you any longer as studies have shown that all this testing just leads to more testing and more uncertainty rather than less.  

Have you had a herpes antibody test since this encounter?  I'm assuming you have, but worth asking.
