I just noticed your age, have Dr do a hormone level test..this would indicate how close you are to menopause ..likely ten years from now , but many fall into menopause in late 40s

a hemmorrhagic follicle is the normal process of ovulation, the part where egg is released still atop the ovary before entering fallopian tube...it could also be containing extra blood due to injury or trauma.
Its sometimes called a "functional cyst" this is what I was told I had yet it kept comming back more often than one would ovulate and was just the left ovary it caused me so much pain and sooo many trips to the ER that I opted to have it removed.
No a hysterectomy is not mandatory,If your close to menopause the fibrroid will dissapear post menopause. Also there are chemical options, some use a birth control pill this is a mild approach it may or may not work. There is a injection option called Leupron ( I am not a Dr you must speak w your Dr about this) Leupron has a side effect for a large percent of women . It is known to cause deep depression, weepiness. May not happen to you, you could always stop if it does. Leupron will put you into a menopause, It works.
I am certain both your conditons are painful, I would urge you to seek atleast one more opinion. Some Drs unfortunatlly are a bit fast to recomend a hysterectomy. This should always be the last choice, the last option. It is a difficult painful surgery. the recovery is long 6 weeks. Once the 6 weeks is done you could resume all normal activities and your pain would be gone.
I would reccomend if you do decide on a hysterectomy to read up on the DaVinci method its the best, safest, least invase type of hysterectomy. Less blood loss, smaller incisions, better success for surgeon with ability to make smaller incisions and cauterisations.

It is possible the size alone causes great pain ( yours are large) uterus removal would cease symptoms..
