Whats causing recent severe lower back pain 23yrs post hysterectomy?

I had a Full LAVH done 23 years ago. For the last week Ive had severe back pain anytime i stand or walk. Pain decreases by a lot right after i empty my bladder and slowly progresses again as my bladder refills. Is this most likely a back, spine or bladder issue?
