swelling in feet

my father's age 54, had a problem of  weakness, swelling in feet,
urine test result say's that albumin+++, what are the main cause,
it may be kidney dissorder.

awaiting suggession

I understand your concern.

Were your dad's doctors able to establish a diagnosis?

Was a urinalysis done to rule out blood in the urine, the presence of an infection, crystals and casts?

Also were blood tests done for blood cholesterol levels , serum creatinine levels and BUN levels? Other tests like a complete blood count and blood albumin levels are also necessary.At this point, a kidney or liver condition need to be ruled out.

The underlying cause for the increase in urinary protein has to be ascertained. Has the physician discussed the possible causes of the proteinuria  with your family?

but his urine-alboumine test +++, need ur advise

Hi,

Your dad needs a follow up consult with his physician.

Is there any improvement with the swelling in the extremities or has the swelling progressed?

The diuretic may be responsible for lowering the BP but this should also help improve the symptoms of edema (swelling). If this does not improve, further evaluation may be needed. This time the liver and the kidneys need to be further evaluated. I understand that both extremities are involved (is this correct)?

Serum albumin level, serum uric acid, 24 hour urine collection ,BUN and Creatinine levels need to be assessed. An ultrasound of the kidneys as well as  determination of liver enzymes may be necessary.

Follow up with the physician prescriber and have your father's response evaluated.Fluid intake may need to be regulated. However , when taking diuretics it is important to note the blood pressure. Do not give when blood pressure is low.Discuss this with your doctor.

I have some additional questions:

How old are you?

Are you male or female?

Based on what you have posted , it is likely that your kidneys are involved.Other factors though have to be considered like liver disease as well as certain factors like diet.However, the focus appears to be that of the kidneys.The edema or swelling in the extremities may also help strengthen the differential of an underlying kidney disorder.

I know you are very worried about this.To allay your fears,you need to discuss this with your physician so you may be guided accordingly.It is necessary to be proactive about this.You may need to be followed up closely.

Dear Mam,
Thanks for ur advise, now docter advise to take 1tab daily of torsimide 10mg & Rabezol IT 1tab daily for 15 days, but weakness & swelling in feet as usual, and bp is 114/76 now, please help

Hi,

Your father is presenting with proteinuria. He should be thoroughly assessed by a nephrologist. A 24 hour urine collection for quantitative urine protein, blood levels of albumin,BUN and creatinine levels, serum electrolytes, a complete blood count, a fasting blood sugar should be determined.A lipid profile should also be taken ( total cholesterol levels).A complete assessment of cardiac function should also be done.

With regards to swelling of the feet,is the swelling something that causes a dent when you press on the swollen area?

Kidney function should be assessed.Nephrotic syndrome as well as chronic kidney disease may cause edema of the extremities and may present as proteinuria.An underlying systemic disease such as diabetes and hypertension should also be managed when present.
