What type of kidney stones is this?

I have just produced another kidney stone. It is 10 mm X 6 mm and it took weeks of pain and hospital visits. I am 40 year/old healthy male and never had any serious medical condition nor any operation. I am on Omnic and antibiotics. Please help. What should I do to reduce the formation of these stones?
