Penile skin not attached at head

I have a very strange problem, havent seen it anywhere and hard to explain without a picture. The skin at the end of my penis is not attached to the penis hole. There is a opening in the skin around the urethral hole and exposes the tissue around the urethra.

That is called your foreskin.  it is not supposed to be attached to the penis head.  It is free to glide back and forth.

The penis head is properly called the "glans penis".
