Perineum tearing, itching and burning?

I have had the Mirena for 4 months. Bled for 3 weeks then random, minimal spotting since. It's great. HOWEVER, once or twice a month, I get perineum tearing, itching and burning. It feels like a yeast infection minus the discharge and smell. I've never had hemorrhoids, but i imagine it feels a little like that too. And to top it all off, I get bumps/pimples on my rear now. Anyone else have this? Solutions or bandaids? I don't want to have to get it removed!
